Amaç: Bu çalışmada Majör Depresif Bozukluk’ta (MDB) sosyal işlev kaybı sağaltımında hem serotonin
hem de noradrenalin üzerinden “ikili eylem” yapacağını düşündüğümüz reboksetin+sertralin
kullanımı ile bir Seçici Serotonin ve Noradrenalin Gerialım İnhibitörü olan venlafaksin’in karşı-
laştırılması amaçlanmıştır.
Yöntem: DSM-IV MDB tanı ölçütlerini karşılayan ve çalışma protokolüne uygun olan 40 hasta çalı
şmaya alındı. Bu hastalar sertralin+reboksetin ve venlafaksin alan iki gruba eşit olarak rastgele
dağıtıldı. Çalışma süresi olan 11 hafta boyunca toplam altı kez değerlendirilen hastalara her değerlendirme
sırasında “Hamilton Depresyon Derecelendirme Ölçeği” (HAM-D), “Montgomery ve Asberg
Depresyon Derecelendirme Ölçeği” (MADRS) ve “Sosyal Uyum Kendini Değerlendirme Ölçe-
ği” (SUKDÖ) dolduruldu.
Bulgular: Her iki ilâç grubunda hastaların HAM-D, HAM-D 7. madde, MADRS ve SUKDÖ değerlerindeki
değişiklikler ilâç öncesi değerlere göre vizit 2’den itibâren (p<0.05) itibâren istatistiksel olarak
anlamlı bulunmuştur. HAM-D 7. madde, MADRS ve SUKDÖ’nün ilâç öncesi değerlere göre zaman
üzerindeki yüzde değişimleri, reboksetin+sertralin ve venlafaksin ilâç grupları arasında istatistiksel
olarak anlamlı farklılık göstermemiştir. Ancak, HAM-D ölçeğinde vizit 3’de venlafaksin ilâç
grubunda yüzde düşüşlerin reboksetin+sertralin ilâç grubuna göre istatistiksel olarak anlamlı bir
şekilde daha fazla olduğu görülmüştür.
Tartışma ve Sonuç: Serotonin ve noradrenalin üzerinden “ikili etki” oluşturduğu bilinen venlafaksin
ile benzer etkinin sağlanabileceği düşünülerek reboksetin+sertralin uygulanan iki hasta grubunun
karşılaştırıldığı bu çalışmada sağaltım yaklaşımları arasında gerek etkinlik gerekse sosyal işlevsellikte
düzelme açısından fark olmadığı bulunmuştur.

Purpose: This study aims to compare the use of reboxetine + sertraline, which according to our
thought would create a “dual effect” via both serotonin and noradrenalin, and venlafaxine which
is a Selective Serotonin and Noradrenalin Reuptake Inhibitor (SNRI) in the treatment of the loss of
social functioning in Major Depressive Disorder (MDD).
Method: The study recruited 40 patients who met the DSM-IV MDD diagnostics criteria and who
were compatible with the study protocol. These patients were randomized into two groups receiving
sertraline+reboxetine and venlafaxine. Throughout the study period of 11 weeks the patients
were evaluated six times, and at each evaluation “Hamilton Depression Rating Scale” (HAM-D),
“Montgomery and Asberg Depression Rating Scale” (MADRS), and “Social Adaptation Self-evaluation
Scale (SASS) were performed.
Findings: In both medicine groups, the changes in HAM-D, HAM-D 7th item, MADRS and SASS
scores was found to be statistically significant starting from the 2nd visit (p<0.05) compared to the
values prior to medication. Percent changes of HAM-D 7th item, MADRS and SASS over time accoryaratding
to pre-medication values did not show any statistically significant difference between the
medicine groups of reboxetine+sertraline and venlafaxine. However, the percent decrease in venlafaxine
group in HAM-D scale was found to be significantly higher compared to the reboxetine+
sertraline group at visit 3.
Discussion and Conclusion: Assuming that a similar effect would be provided as that of venlafaxine
which is known to create a “dual effect” via serotonin and noradrenalin, two groups of patients
on medication of reboxetine+sertraline were compared in this study. However, it was found
that there was no difference between the approaches of treatment as far as efficacy and remission
in social functioning were concerned.
