Dar terapötik indeksi nedeniyle lityum kullanımında toksisite yaygın olarak görülmektedir. Lityumun terapötik ve toksik dozları birbirine oldukça yakındır. Lityum, vücutta değişik sistemlerde, çeşitli yan etkilere sebep olabilir. Nörotoksisite, lityum tedavisinin en sık görülen, ancak muhtemelen en az tanınan yan etkilerinden biridir. Lityum ilişkili nörotoksisite, tedavinin akut fazında görülebildiği gibi uzun dönemde de karşımıza çıkabilir. Serum lityum seviyesi, nörotoksisitenin kesin göstergesi olmadığı gibi, semptomların ağırlığı ile de tam olarak ilişkili değildir. Bu nedenle, klinisyenler lityum başlarken, hastaları kalıcı nörolojik sekel oluşumundan korumak için, tremor ve serebellar bulgular gibi klinik işaretler konusunda uyanık olmalı ve ileri yaş ve ilaç kombinasyonu gibi risk faktörlerini göz önünde bulundurmalıdırlar.

Toxicity is widely seen in lithium use related to its narrow therapeutic index. Therapeutic and toxic dosages of lithium are considerably close to each other. Lithium may cause several side effects on various systems of body. Neurotoxicity is one of the most frequent but probably the least diagnosed adverse effect of lithium theraphy. Lithium-associated neurotoxicity may be seeen not only in acute phase but also in long term period of lithium use. Serum lithium level is not a definite predictor of neurotoxicity and it is not explicitly associated with the severity of symptoms. Hence, when prescribing lithium, clinicians should be vigilant to clinical signs like tremor and cerebellar findings and consider risk factors like older age and drug combination to prevent patients from development of irreversible neurological sequela.
