Amaç: Birçok çalışmada kronik ağrı seyrinde psikiyatrik bozukluklara sık rastlandığı gösterilmiştir. Depresyon bunlardan en fazla karşılaşılanlardan biridir. Ancak hangi mekanizma ile ortaya çıktığı henüz tam olarak bilinmemektedir ve çoğu zaman detaylı değerlendirilmediği için gözden kaçar. Kronik mekanik bel ve boyun ağrısı tanısı alan hastalarda depresyon düzeyinin ve bunun ağrı şid­deti ve süresi ile ilişkisinin araştırılması.
Yöntem: Çalışmaya kronik bel ve boyun ağrısı olan toplam 50 (25 bel ağrısı, 25 boyun ağrısı) has­ta alındı. Ağrı şiddetini ölçmek için Vizüel Analog Skala (VAS), depresyon şiddetini ölçmek için Ha­milton Depresyon Ölçeği (HDÖ) kullanıldı.
Bulgular: Yaş ortalaması ve kadın/erkek dağılımı açısından gruplar arasında anlamlı bir fark yok­tu. Ağrı şiddeti ve depresyon düzeyi açısından iki grup arasında anlamlı fark bulunmadı. Ağrı sü­resi bel ağrılı grupta boyun ağrılı gruptan anlamlı olarak daha uzundu (p<0.001). Her iki grupta depresyon şiddeti ile ağrı süresi arasında pozitif korelasyon mevcuttu (r=0.621, p<0.001, r=871, p<0.001).
Tartışma: Çalışma sonuçlarımıza göre kronik bel ve boyun ağrısı olan hastalarda depresyon düze­yi ağrı şiddetinden çok ağrı süresi ile ilişkilidir.
Sonuç: Uzun süreli ağrı şikâyeti olan bel ve boyun ağrılı hastaların depresyon yönünden de değer­lendirilmesi gerektiği düşüncesindeyiz.

Objective: Depression is a common but frequently underdiagnosed problem in patients with chro­nic pain. However, the role of depression in chronic pain is not well revealed. The purpose of this study was to compare the severity of depression in patients with low back and neck pain and to assess its relationship with the duration of the disease and the severity of the pain. Method: This study was conducted between August 2005 and October 2005 in the department of Physical Medicine and Rehabilitation in Harran University Research Hospital. Fifty patients were di­agnosed as having chronic mechanic low back pain and neck pain. The severity of depression and pain was measured by Hamilton Depression Scale (HAM-D) and Visual Analogue Scale (VAS), con­secutively.
Findings: There were no significant differences in case of age, female/male ratio, severity of pain and depression between two groups. The duration of the disease in patients with low back pain was longer than those of with neck pain (p < 0.001). There was a significant correlation between the duration of low back pain and the severity of depression (r = 0.621, p <0.001). Similarly, we fo­und a significant correlation between the duration of neck pain and the severity of depression (r = 871, p< 0.001).
Discussion: The present results indicate that there were relationship between depression and the duration of the disease in patients with chronic low back pain and neck pain. Conclusion: We suggest the severity of depression should be evaluated in patients with chronic low back and neck pain.
