İntihar şizofrenili hastalarda önde gelen ölüm nedenidir. Genel nüfus ile karşılaştırıldığında şizofrenili kişiler 9 kez daha fazla intihar nedeni ile ölmektedir. Şizofrenili kişiler arasında intihar davranışı oranı %20-40 olarak tahmin edilmektedir. Depo hastanelerin tasfiye edildiği ülkelerde yatarak tedavi gören şizofrenili hastaların hastanelerdeki günlük yatak işgali 25 yıl öncesine göre %60 oranında azalmıştır. Bunun anlamı daha önce hastane lerde bulunan şizofrenlerin 2/3'ünün daha az süpervizyon alması ve daha az gözlem altında tutulmasıdır. Ayrıca şizofreni tanısı konan hasta ların artık daha çok toplum içinde sağaltılması, ailelerin yaşadığı sorunların artmasına neden olmaktadır. Aşağıda intihar girişimi sırasında üstüne düştüğü kişinin ölümüne sebep olan bir şizofreni olgusu sunulmuştur. Olgunun diğer bir özelliği iki kız ve bir erkek kardeşinin de şizofreni hastası olması ve sosyal desteklerinin bulunmamasıdır. Kız kardeşlerden biri tamamlanmış intihar sonucu ölmüş, diğer kız kardeş ise kendi çocuğunu öldürmüştür. Bu olgu bağlamında süreğen ruhsal hastalığı olan birey ve ailelerinin sorunları toplum psikiyatrisi temelinde tartışılmıştır.

Suicide is the leading cause of premature death among persons with schizophrenia. Compared to the general population, persons with schizophrenia are nine times more likely to die by suicide. The estimated rate of suicidal behavior among persons with schizophrenia is between 20% and 40%. In countries where decentralization, the number of schizophrenia bed days in hospital setting is now only one-third of figures from 25 [year]s ago. This mean that patients representing two-thirds of all schizophrenia bed days are now under a lower degree supervision and observation than formerly. In additional, treating patients with schiz ophrenia mostly in the community causes more problems for the families. We report a female schizophrenic patient who caused death of other person during suicide attempt with jumped from heights. The other characteristic of case; she had two sister and one brother with schizophrenia and no social support. One of the sisters committed suicide and the other sister killed her son. In the light of this case; we dis cussed problems of individuals with chronic psychiatric disorders and their family based on community psychiatry.
