Deliryum, hemşirelerin genel hastane ortamında sıklıkla karşılaştıkları bir durumdur. Deliryum yaşlılarda gençlerden daha sık görülür. Genellikle hastaneye yatışın ikinci veya üçüncü günü, konfüzyonun ortaya çıkabile ceğinin habercisi olabilir. Deliryumun temel özel liği, bilinçlilik durumundaki bozulma ve belirgin bilişsel sorunlardır. Deliryum çok kısa bir süre içinde gelişir, hafiften şiddetliye doğru sınıf landırılır, zamana ve yere ilişkin ileri derecede oryantasyon bozukluğu ile kendini gösterir. De liryuma neden olan bozukluk saptanıp, düzel tilirse, tamamen iyileşme mümkün olur. Bununla birlikte bozukluk ısrarlı ve düzeltilmemiş ise demans ve ölüm gibi geri dönüşsüz bir duruma neden olabilir. Deliryumu oluşturabilecek en temel nedenlerden bazıları enfeksiyonlar, postoperatif durumlar ve hipoksi yaratan koşullar, metabolik bozukluklar, ilaç yoksunlukları ve zehirlenmeliridir. Bu makalenin sunuluşundaki amaçlar; deliryumdaki bireyin üç kritik gereksini mini saptayarak bunları hemşirelik tanıları ile ifade etmek ve birey için temel olan somatik ve psikoterapötik girişimleri özetlemektir.

Nurses frequently encounter delirium in the general hospital setting. Delirium occurs more frequently in elderly than in younger clients. Generally, the second or third hospital day may herald the onset of confusion. The essential fea ture of delirium is a disturbance in conscious ness and is generally marked by cognitive diffi culties. The clinical manifestations of delirium develop over a short period. Delirium is charac terized by progressive disorientation to time and place and can be classified as mild to severe. If the underlying disorder is corrected and reversed, complete recovery is possible. If, how ever, the underlying disorder is not corrected and persists, can lead to irreversible changes, such as dementia and dealth. Some of the most common causes of delirium include infections, postoperative states, metabolic abnormalities, hypoxic conditions, drug withdrawal states, and drug intoxications. The objectives of presenta tion of this article is to discuss three critical needs of a person with delirium and state them in terms of a nursing diagnosis and summarize the essential somatic and psychotherapeutic interventions for a client with delirium.
