Hippokampal sklerozu bulunan temporal lob epilepsi hastalarında selektif-amygdalohippokampektomiden önce ve sonra sözel bellek performansı

Amaç: Bu çalışmada, hippokampal sklerozu bulunan temporal lob epilepsi (TLE-HS) hastalarında operasyon öncesi ve sonrası sözel bellek performansları incelenmiş, operasyonun ve lateral izasy onun etkileri araştın İmi ştır. Son olarak da, yaş ve eğitim süresinin sözel bellekle olan ilişkisi incelenmiştir.
Yöntem: Çalışma kesitsel araştırma metoduyla gerçekleştirilmiş olup, operasyona girmeyi bekleyen hippokampal sklerozu bulunan 20 temporal lob epilepsi hastası, ameliyat olmuş 21 hasta ve her iki epilepsi grubuyla demografik değişkenler bakımından eşleştirilmiş 20 kontrol katılımcısı çalışmaya alınmış ve katılımcıların her birine California Sözel Öğrenme Testi (CSÖT) uygulanmıştır.
Bulgular: Veri analizine göre, her 3 grup arasında serbest hatırlama, karışma sayısı, ayırt etme ve yalancı pozitif sayısında anlamlı farklar gözlenmiştir. Operasyona girmemiş grubun, girmiş gruba oranla daha iyi serbest hatırlama performansı gösterdiği ve daha fazla sayıda perseverasyon yaptığı bulunmuştur. Lateralizasyon açısından İncelendiğinde ise, sağ TLE-HS grubu, sol TLE-HS'lilere oranla daha iyi serbest hatırlama ve anlamlı oranda daha az karışma göstermişlerdir. Eğitim süresi ile CSÖT'nin alt Ölçeklerinden serbest hatırlama ve ayırt etme arasında pozitif bir ilişki bulunurken, karışma ve yalancı pozitif sayısı ile eğitim süresi arasında ters yönde bir ilişki bulunmuştur.
Tartışma: Operasyon sonrası grubun, kontrol grubundan ve operasyon öncesi gruptan daha kötü bîr sözel bellek performansı gösterdiği ve sol temporal bölgedeki bir epileptojenik odağın, sözel bellek performansını olumsuz yönde etkilediği bulunmuştur.
Verbal Memory Performance of Patients with Temporal Lobe Epilepsy and Hippocampal Sclerosis before and after Selective-Amygdalohippocampectomy
Objective: The main aim of the present study was first to assess the diffferences between operated and non-operated patients with temporal lobe epilepsy and hippocampal sclerosis (TLE-HS) in terms of the verbal memory. Second, the effect of lateralization on verbal memory was examined. Finally, the relations between age- verbal memory performance and [year]s of education-verbal memory performance were evaluated.
Method: This study was performed In a cross-sectional design; twenty patients who were waiting for surgery (pre-op group), 21 patients who had already underwent surgery (post-op group) and 20 matched normal controls who were matched with both of the epilepsy groups In terms of demographic variables participated in the study. California Verbal Learning Test (CVLT) was administered to the subjects.
Results: The results showed that the three groups were significantly different in terms of recall, number of intrusions, discrimination, and false positives. Pre-op group had a better recall performance and more perseverations than the post-op group. In terms of lateralization, right sided TLE-HS group had a better recall performance and significantly less number of intrusions than left sided patients. Also, there is positive relation between recall and [year] of education and discimination and [year] of education whereas there is a negative correlation between intrusions and [year] of education and false positives and [year] of education.
Conclusion: Post-op group had significantly lower verbal memory scores than the pre-op and control groups whereas a left epileptogenic focus in the left temporal lob was found to be affected the verbal memory negatively
