Amaç: Türkiye'de filtreli paket sigara kullanımı İle ilgili çok sayıda çalışma olmasına karşın sarma tütün kullanımı ile ilgili sadece bir çalışma vardır. Hâlbuki Doğu ve Güneydoğu Anadolu Bölgesi'nde tütünün sarma tütün biçiminde kulla­nıldığı ve bu bölgelerde tütün pazarlarının olduğu bilinmek­tedir. Bu toplumsal kesim, tütün kullanımı ile ilgili bilgi ve istatistiklerin dışında kalmaktadır. Bu çalışmada Şanlıurfa'da kırsal alanda, tütün kullanma biçimlerinin ve bağımlılık dü­zeylerinin araştırılması amaçlanmıştır.
Yöntem: Şanlıurfa'da Siverek ilçesine bağlı 600 nüfuslu bir köyde, 15 yaş üstünde olan tüm bireylerin oluşturduğu 299 kişi çalışmaya alındı. Çalışmacılar tarafından oluşturulan Anket Formu ve nikotin bağımlılık düzeyini ölçen Fagerstrom Nikotin Alışkanlık Ölçeği uygulandı.
Bulgular: Çalışmaya katılan 299 kişiden %55.5'i erkek (n=166), %44.5'i kadındı (n=133). Yaş ortalaması 35.7 ± 15.8 yıl idi. Okuma yazma bilmeme oranı %38 idi. Hayatı boyunca bir dönem tütün içen oranı %47.2 (n=141), halen tütün kulla­nan %38.8 (n=116) oranındaydı. Tütün kullananlardan % 93'ü sarma tütün %7'si paket tütün kullanıyordu. Günde 20'den fazla sarma sigara içme %58.2 olup, %13.6'sı 30 yıldan uzun süre sarma sigara kullanmıştı. Olguların %87.3'ü tütüne filtre takınaksızın, %84.4'ü ağızlık kullanmadan %21'i tütünü sarıp tabakada biriktirerek, %78'i her içim için yeniden sigara sar­maktaydı. Aralıklı olarak paket sigara da kullanma oranı %17 olup, %81.7'sinde ailede sarma tütün kullanan başka biri de vardı. Sarma tütün tercih nedeni olarak %49.1 'i daha ucuz ol­masını, %32.1'i daha lezzetli olması olarak göstermekteydi. Sarma tütün kullananların %87.5'i sarma tütünün daha zarar­lı olduğunu düşünürken, %12.5'i paket sigaranın daha zararlı olduğunu düşünmekte idi.- Nikotin alışkanlık ölçeğinden alı­nan puan ortalaması 8.79 ± 1.46 olup, bu ölçeğe göre sarma tütün kullananların %79.6'sı fiziksel nikotin bağımlısı idi.
Sonuç: Şanlıurfa'da kırsal alanda tütün kullanımının en yaygın biçimi sarma tütün kullanımınıdır. Tütünün gelenek­sel kullanım şeklinin sarma tütün şeklinde olması ve sarma tütünün daha ucuz olması bu durumun en önemli belirleyi­cileridir.

Objective: There are numerous studies about smoking filter-tipped cigarettes in Turkey. However, there Is only one study about smoking rolled tobacco according to our know­ledge. Smoking rolled tobacco is common in Eastern and Southeastern Turkey. The purpose of this study was to inves­tigate soclodemographic characteristics, smoking habits and dependence levels in people who smoke rolled, tobacco in the rural area of Sanliurfa.
Methods: This was study performed in a village In Sive­rek district of Sanliurfa. The village had a total population of 600. Two hundred ninety-nine subjects over 15 [year]s of age were recruited in the study. Fagerstrom Tolerance Scale and a questionnaire about smoking rolled tobacco which were prepared by research team were applied to all subjects.
Results: This study included 299 subjects (55.5% male, 44.5% female). The mean age of subjects was 35.7 ± 15.8 [year]s old. Thirty-eight percent of subjects were Illiterate. Fourty-seven percent of subjects (n=141) had used tobacco at least one period of their lifetime. Thirty-eight percent of subjects (n-116) were currently smoking tobacco. Ninetythree percent of subjects were smoking rolled tobacco, while only 7% were smoking cigarettes. 58.2% of subjects smoked more than 20 cigarettes a day. 13.6 % of subjects smoked more than 30 [year]s, 84.4% smoked tobacco without filter, 84.4% smoked without a cigarette holder. Whereas 21% of subjects collect rolled tobacco in a cigarette case, others roll tobacco for each smoke. 17% of subjects sometimes smoke filter-tipped cigarettes besides rolled tobacco. 81.7% of sub­jects had a family member who smoke rolled tobacco. The reason for smoking rolled tobacco was its low cost compa­red to filter-tipped cigarettes for 49.1% of subjects. 87.5% of subjects thought that rolled tobacco was more harmful than filter-tipped cigarettes. The average score of Fagerstrom To­lerance Scale was 8.79±1.46 in the study group. These results suggested that 79.6% of subjects who smoke rolled tobacco had physical dependence on nicotine.
Conclusion: Rolled tobacco smoking is the most com­mon form of tobacco use in the rural area of Sanliurfa. Tra­ditional usage and the low cost of rolled tobacco are the Important factors which determine smoking rolled tobacco in Sanliurfa.
