Yapısal beyin görüntüleme çalışmaları şizofreniklerde yan ve üçüncü ventriküllerde genişleme lehine güçlü kanıtlar taşımaktadır, ancak bu genişleme azdır ve normallerle örtüşme söz konusudur. Lokalize yapısal değişiklikler, daha çok solda ve subkortikal yapılarda olmak üzere, temporolimbik yapılarda görülmektedir. Defısit belirtiler ve yapısal bozukluklar arasında ilişki bulunduğuna dair bazı kanıtlar vardır.
Bazı işlevsel beyin görüntüleme çalışmalarında istirahat halindeki hastalarda hipofrontalite saptanmıştır, ancak test aktivasyonu sırasında hipofrontaliteye ilişkin kanıtlar daha güçlüdür. Yine negatif belirtilerle hipofrontalite arasında daha güçlü bir ilişkiye rastlanmaktadır. Çalışmaya alınan hastalar ve kullanılan tekniklerdeki farklılıklar, sonuçların yorumlanmasını güçleştirmektedir.

Technology provides us a window that we can look through into the brain. Among the psychiatric disorders, schizophrenia is the one that has been most often studied by brain imaging technics.
Structural brain imaging studies suggest strong evidence for the enlargement of lateral and third ventricles in schizophrenics, however this is modest and there is a large overlap with the normals. Temporal limbic structures, more on the left and subcortical, seem to be sites of localised structural abnormality. There is some evidence for correlation between the presence of deficit symptoms and structural findings.
A number of functional brain imaging studies estimated hipofrontality under resting conditions, but there is stronger evidence for hipofrontality during task activation. The evidence is also stronger for a selective association between hipofrontality and negative symptoms. The differences in patient samples and imaging techniques make the results more difficult to interpret.
