Gelişimsel özellikler nedeni ile mani ve hipomani belirtileri erişkinler ve çocuklar arasında farklılaşabilir. Bu nedenle süreğen ve şiddetli ve çabuk öfkelenme ya da huzursuzluğu, aşırı hareketliliği ve anormal duygudurumu (genellikle üzüntü ve/ya da öfke) olan çocuklarda en uygun tanıyla ilgili belirgin bir anlaşmazlık mevcuttur. Bu alandaki araştırmacılar tarafından, fenotipler sistemi bu tanısal karmaşayı çözmek amaçlı öne sürülmüştür. Bu araştırmacılar her fenotipe ilişkin çocukların sistematik değerlendirilmesi ve uzunlamasına izlenebilmesi için "geniş" ve "dar" fenotipler tanımlamışlardır. Geniş fenotipi süreğen ve döngüsel hastalığı olup maninin işaret belirtilerini içermeyen, fakat daha dar fenotiplerin şiddetli çabuk öfkelenme ya da huzursuzluk ve aşırı uyarılmışlık belirtilerini paylaşan hastalar göstermektedir. Biz bu yazıda geniş fenotip tanı ölçütlerini karşılayan bir olgumuzun izlem sürecinin ve gelişim öyküsünün tartışılmasını amaçladık. Döngülerin varlığı ve belirtilerin özelliği çocukluk manisindeki fenotiplerin ayırımında kullanılabilir. Bu sistemin kullanılabilirliği ve güvenilirliği izleyen çalışmalarda test edilebilecektir.

Presentation of mania and hypomania may differ between children and adults because of the developmental differences. For that reason, there is a considerable disagreement about the most appropriate diagnosis for children with chronic and severe irritability, hyperactivity and abnormal mood (usually sadness and/or anger). Researchers in this field suggested a phenotype system to solve the diagnostic controversial problems. These researchers defined "narrow" and "broad" phenotypes for juvenile mania aiming children meeting the criteria for each phenotype could be assesed systemically and followed longitudinally. The broad phenotype is exhibited by patients who have a chronic, non episodic illness which does not include the hallmark symptoms of mania but shares the narrower phenotypes of the symptoms of severe irritability and hyperarousal In this article we aimed to discuss the follow-up period and the development history of a case who meets the criteria of broad phenotype. The presence of episodes and symptoms can be used in differentiating the clinical phenotypes of juvenile mania. The utility and validity of this system can be tested in subsequent researchs.
