Amaç: Cinsel işlev bozukluğunun anksiyete belirtileri ile birlikte olması bozukluğun seyrini ve tedavisini etkileyebilir. Aynı zamanda, sosyokültürel özellikler açısından Türk toplumunda cinsel işlev bozukluklarının, erkeklik imajı ile bağlantısının olması ciddi bir travma etkeni ve stresördür. Buna bağlı olarak, erkelerde cinsel işlev bozukluğunun anksiyete şiddeti ile ilişkisi değerlendirmek üzere bu çalışma plânlanmıştır. Yöntem: Cinsel işlev bozukluğu olan 45 erkek hastanın sosyodemografik özellikleri ve anksiyetesi incelenmiştir. Çalışmaya alınan vak’aların 21’ine (%46.7) cinsel uyarılma bozukluğu (erektil disfonksiyon), 4’üne (%8.9) orgazmla ilgili bozukluk (prematür ejakülasyon), 20’sine (%44.4) birden fazla cinsel işlev bozukluk tanısı kondu. DSM-IV’e göre hiçbir hastaya psikopatolojik tanı konmadı. Bulgular: Olguların yaş ortalaması 32.56±9.74 bulundu. Cinsel işlev bozukluğu olan 45 hastaya SCID (DSM-III-R için yapılandırılmış Klinik görüşme Ölçeği) ve Spielberger Durumluk ve Sürekli Anksiyete Envanteri (STAI) uygulandı. Spielberger sürekli anksiyete için genel ortalama 49.18±6.72 derece iken, durumluk anksiyete için 43.24±9.82 bulundu. Durumluk anksiyete yönünden cinsel sorunun şiddeti arasında (x2=6.47, sd=2, p<0.05) ve sürekli anksiyete yönünden olumsuz etkileyen faktörler arasında fark bulundu (x2=15.89, sd=8, p<0.05). Tartışma: Cinsel işlev bozukluğu olan erkeklerin durumluk ve sürekli anksiyete şiddetleri “normâl” düzeyde saptanmış, patolojik anksiyete kabûl edebileceğimiz yüksek anksiyete düzeyi daha düşük bulunmuştur. Cinsel sorunun şiddetinin durumluk anksiyeteyi, olumsuz faktörlerin ise sürekli anksiyeteyi etkilediği saptanmıştır. Sürekli anksiyete ortalamasının durumluk anksiyete ortalamasından daha yüksek olması da dikkat çeken bir bulgudur. Sonuç: Cinsel işlev bozukluğu olan hastalarda anksiyete şiddetinin genel popülasyondaki “normâl değerlerden” yüksek olması anksiyete düzeyinin önemsenmesi gerektiğini göstermektedir. Bu konuyla ilgili daha fazla sayıda hasta ile yapılacak karşılaştırmalı çalışmalar tedaviyi olumsuz etkileyebilecek anksiyetenin daha iyi değerlendirilmesini sağlayacaktır.

Objective: Sexual dysfunction with anxiety symptoms should affect its course and treatment. However, male sexual dysfunction with regard to sociocultural characteristics of Turkish people connected with masculinity is a trauma factor and stressor. Therefore, aim of this study is to evaluate the relationship between sexual dysfunction and anxiety. Method: Characteristics of male sexual dysfunction were investigated in this study involving a sample of 45 outpatients whose sociodemographic characteristics and anxiety is described. 21 (46.7%) patients were suffering from arousal disorders (erectile dysfunction), 4 (8.9%) of orgasm disorders (premature ejaculation) and 20 (%44.4) men complained of multiple dysfunctions. None of the patients suffered from a psychopathological disorder according to DSM-IV. Findings: Mean age was 32.56±9.74. 45 patients with sexual dysfunction were used SCID (Structured Interview for DSM-III-R) and Spielberger State-Trait Anxiety Inventory (STAI). According to our data, mean score was 43.24±9.82 for state anxiety and was 49.18±6.72 for trait anxiety. There were statistically significant difference between state anxiety and severity of sexual dysfunction (x2=6.47, sd=2, p<0.05), and between trait anxiety and factors that effect negatively to sexual disorders (x2=15.89, sd=8, p<0.05). Discussion: We found that state and trait anxiety severity of men with sexual dysfunction was “normal” level and high anxiety level that was accepted pathological anxiety was lower. While severity of sexual problems is affected state anxiety, negative factors are affected trait anxiety. Interestingly, another finding was found that mean score of trait anxiety score was higher than state anxiety. Conclusion: We concluded that state and trait anxiety of the patients with sexual dysfunctions were higher than the preceding normal levels in the general population. Anxiety in sexual dysfunction should be studied across different studies with larger samples, with control groups of similar demographic features, and in longitudinal follow-up designs. Future research is needed to evaluate the effects of anxiety on the recovery of sexual dysfunction.
