Amaç: Bu çalışmada iki cilt hastalığı olan vitiligo ve kro­nik ürtiker hastalarında stres yaratan yaşam olaylarının niteliği ve şiddetinin araştırılması amaçlanmıştır. Yöntem: Çalışmaya istanbul'da dört büyük hastanenin Dermatoloji Poliklinikleri'ne ayaktan başvuran, vitiligo ve kronik ürtiker tanıları almış, 16-60 yaş arası 50'şer hasta alınmıştır. Kişilere tarafımızdan hazırlanan sosyodemografik özelliklerin incelendiği soru formu. Stresli Yaşam Olaylarını Tarama Formu (Stressful Life Experiences Screening=SLES) ve SCID-I uygulanmıştır. Bulgular: Hasta gruplarında, kontrol grubuna göre anlamlı derecede daha fazla ciddi kaza, yaralanma; eş, çocuk ve yakın arkadaş ölümü; çocuklukta ve erişkinlikte fiziksel şiddet ile karşılaşmış ve tanık olmuşlar; henüz kimseye bahsetmedikleri aşırı derecede stresli bir olay yaşamışlardır. Her iki cilt hastalığı grubunda, SCID-I tanıları arasında sadece distimik bozukluk ve genelleşmiş anksiyete bozukluğu (GAB) anlamlı olarak farklı bulunmuştur. SCID-l'e göre vitiligo grubunda distimik bozukluk %26, GAB %6; kronik ürtiker grubunda distimik bozuk­luk %46, GAB %30 oranlarında bulunmuş; ve her iki cilt hastalığı grubunda kronik ürtiker lehine distimik bozuk­luk ve GAB oranları sıklığı, anlamlı olarak daha fazla bulunmuştur. Sonuç: Çalışmamız stres yaratan yaşam olaylarının, vitiligo ve kronik ürtiker gibi iki farklı etyopatogenezi olan cilt hastalığında bazı SCID-I tanıları geliştirecek kadar önemli olduğunu vurgulamaktadır.

Objective: This study was designed to investigate the quality and severity of stressful life events in two kinds of skin disorders vitiligo and chronic urticaria. Method: The study population was recruited from dermatology out­patient clinics at four major hospitals in Istanbul. 50 vitiligo, 50 chronic urticaria patients, aged 16-60 [year]s were compared with age- and sex- matched 50 healthy controls. The sample completed the sociodemographical data form. Stressful Life Experiences Screening (SLES) and the SCID-I. Results: Among SCID-I diagnoses, only the rates of dysthymic disorder and generalized anxiety disorder (GAD) were found statistically significantly dif­ferent between the two skin disorders. The rates of dys­thymic disorder were 26%, GAD were 30% in chronic urticaria group in which the difference between the two skin disorders group were statistically significant in which the rates of SCID-I diagnoses were significantly higher in chronic urticaria group than in vitiligo group. Both of the patient groups experienced significantly higher rates of severe accident, injury; death of a spouse, child, close friends exposition to physical violence in childhood and adulthood, and a serious stressful event that was declared to nobody than control group. Conclusion: Our study emphasizes the importance of stressful life events leading to develop some SCID-I diag­noses in vitiligo and chronic urticaria which are skin dis­eases with different etiopathogenesis and clinical mani­festations.
