Psikotik bozuklukların tedavisinde depo antipsikotik ilaç (flufenazin dekanoat) kullanımı neden hâlâ önemlidir?: Klinik sonuç açısından bir inceleme

Amaç: Tedavi uyumsuzluğu olan şizofreni ve sanrılı bozukluk hastalarına depo antipsikotik ilaç (DAPİ) tedavisi uygulanarak klinik sonucun değerlendirilmesi amaçlanmıştır.
Yöntem: Kocaeli Üniversitesi Tıp Fakültesi Hastanesi Psikotik Bozukluklar Polikliniğinde, 1 Ağustos 2003 ile 1 Ağustos 2004 tarihleri arasında, DSM-IV'e göre şizofreni ve sanrılı bozukluk tanısıyla izlenen hastalardan son 6 ayda tedavi uyumsuzluğu olan 21 hasta flufenazin dekanoat depo tedavisine alındı. Hastaların çalışma öncesinde kullandıkları varsayılan klorpromazin eşdeğeri antipsikotik ilaç dozları, Pozitif ve Negatif Sendrom Ölçeği (PANSS], Klinik Genel Değerlendirme Ölçeği (CGI / KGDÖ] ve işlevselliğin Genel Değerlendirmesi Ölçeği (İG-DÖ] puanları kaydedildi. Flufenazin dekanoat ampul 25 mg, birinci ay haftada bir, ikinci ay iki haftada bir, üçüncü ay üç haftada bir kas-içi olarak uygulandı. DAPİ uygulamasından sonraki birinci ve üçüncü ayda aynı ölçeklerden alınan puanlar ve üçüncü ayda kullanılan ilaç dozu kaydedildi. Önceki PANSS, KGDÖ, İGDÖ puanları ile birinci ve üçüncü aydaki puanlar karşılaştırıldı.
Bulgular: DAPİ uygulamasına başlamadan önce 10 hasta 321.60±214.80 mg/g klorpromazin eşdeğeri antipsikotik ilaç kullanıyor 8 hasta ilaçlarını düzensiz kullanıyor, 3 hasta da hiç ilaç kullanmıyordu. Yirmi bir hasta 3 aylık DAPİ tedavisini tamamladı. PANSS puan ortalaması başlangıçta 68.85±13.42, birinci ayda 49.85±7.39 (p=0.001), üçüncü ayda 40.38±6.52 (p=0.001) idi. KGDÖ puan ortalaması başlangıçta 4.71 ±0.78, birinci ayda 3.76±0.70 (p=0.001), üçüncü ayda 2.90±0.94 (p=0.001) idi. İGDÖ puan ortalaması başlangıçta 55.95+8.15, birinci ayda 63.09±6.97 (p=0.001), üçüncü ayda 67.38+5.15 (p=0.001) idi. Tüm hastalar üçüncü ayda 125 mg/g klorpromazin eşdeğeri DAPİ kullanıyordu.
Sonuç: Tedavi uyumsuzluğu olan şizofreni ya da sanrılı bozukluk hastalarında depo antipsikotik ilaç kullanımı klinik sonuç açısından çok önemli bir tedavi seçeneğidir.
Why depot antipsychotic drug use in the treatment of psychotic disorders is still important? An investigation regarding clinical outcome
Objective: Clinical outcomes of depot antipsychotic drug (DAPD1 use for noncompliant patients with schizophrenia and delusional disorder were evaluated in this study.
Method: Between August 1st 2003 and August 1st 2004, 21 treatment noncompliant patients with schizophrenia and delusional disorder, diagnosed according to DSM-IV at the University of Kocaeli, School of Medicine, Psychotic Disorders Outpatient Clinic, were enrolled into this study of depot antipsychotic drug (fluphenazine decanoate] treatment (DAPD1. Chlorpromazine equivalent antipsychotic drug doses assumed to be used by the patients and the Positive and Negative Syndrome Scale (PANSS] scores, Global Assessment of Functioning (GAR scores, and Clinical Global Impressions severity scale (CGIssl scores were recorded. Fluphenazine decanoate 25 mg was injected to all patients intramuscularly every week in the first [month], every two weeks in the second [month], and every three weeks in the third [month]. Patients were evaluated using the same scales in the first and third [month]s, and drug doses were recorded in the third [month]. Comparisons between the baseline, first and third [month] scores of the mentioned scales were carried out.
Results: Ten patients were seem to use 321.60±214.80 mg/day chlorpromazine equivalent antipsychotic drugs, eight patients were using their drugs irregularly, and three patients were not using any antipsychotic drugs before DAPD use. Twenty-one patients completed the three- [month] long study. The mean PANSS scores were 68.85±13.42 at baseline, 49.85±7.39 (p=0.001) in the first [month], and 40.38±6.52 (p=0.001) in the third [month]. The mean CGIs scores were 4.71±0.78 at baseline, 3.76±0.70 (p=0.001) in the first [month], and 2.90±0.94 (p=0.001) in the third [month]. The mean GAF scores were 55.95+8.15 at baseline, 63.09±6.97 (p=0.001) in the first [month], and 67.38+5.15 (p=0.001) in the third [month]. All patients were using 125 mg/day chlorpromazine equivalent depot antipsychotic drug in the third [month]. Conclusions: Depot antipsychotic drug use in noncompliant patients with schizophrenia or delusional disorder is an important treatment choice regarding clinical outcome.
