Karotis Endarterektomi Esnasında Gelişebilen Kraniyal ve Periferik Sinir Hasarının Araştırılması

ÖZET
Amaç: Karotis cerrahisi uygulanan hastalarda, zaman zaman vagus, hipoglossus ve fasiyal sinir gibi kraniyal ve periferik sinirlerin hasarı bildirilmektedir. Çalışmamızda, karotis endarterektomisi uyguladığımız hastalarımızda gelişebilecek sinir hasarlarının objektif bir yöntem olan elektrofizyolo-jik çalışma ile gösterilmesi amaçlandı.
Yöntem: Aynı standart yöntemle karotis endarterektomi + patch plâsti operasyonu yapılan toplam 18 hastanın, operasyondan ortalama 1 ay sonra iki taraflı kraniyal ve periferik sinir elektrofizyolo-jik çalışması yapıldı. Ayrıca vagus sinirinin değerlendirilmesi amacıyla, bu hastalara ve sağlıklı 20 kontrol bireye istirahatte ve hiperventilasyon esnasında R-R interval değişkenliği ölçümü yapıldı. Bulgular: On birinde (%61) sol karotis lezyonu olan hastaların 15'ine (%83) endarterektomi sonrası sentetik politetrafloroetilen, üçüne de (%17) safen ven ile patch plâsti işlemi uygulandı. Hastaların hiçbirinde perioperatif strok gibi majör bir komplikasyon veya mortalite gözlenmedi. Hastaların birinde preoperatif dönemde de düşünülen bilateral karpal tünel sendromu, elektrofizyolo-jik olarak doğrulandı. Preoperatif inme sekeli olan üç hasta dışındaki bir hastada (%5.5) fasiyal sinirin parsiyel lezyonu saptandı. Diğer hastalarda postoperatif dönemde kraniyal veya periferik sinir hasarı saptanmadı. Operasyon tarafı ve karşı tarafın ölçülen sinir ileti hızı ve amplitüdleri arasında istatistiksel olarak anlamlı fark gözlenmedi. Ayrıca, istirahatte ve hiperventilasyonda alınan R-R interval değişkenliği ölçümlerinde, kontrol grubu ve karotis endarterektomili hastalar arasında da istatistiksel olarak anlamlı fark yoktu.
Tartışma: Karotis endarterektomisi, genellikle inmenin önlemesi amacıyla yapılmakta fakat bazen istenmeyen komplikasyonlara yol açabilmektedir. Fasiyal sinir dalları, vagus ve hipoglossus sinir yaralanmaları sıkça bildirilmektedir. Yapılan çalışmaların hemen tamamında lezyon, şüphelenilen hastaların incelenmesiyle ortaya konulmuştur. Hastaların tamamının elektrofizyolojik olarak incelendiği bir çalışmaya literatürde rastlanmamıştır. Vagus sinirini de içeren bâzı kraniyal ve periferik sinirlerin elektrofizyolojik olarak incelendiği çalışmamızda bir hastada gelişen parsiyel fasiyal sinir lezyonu dışında başka bir sinir hasarına rastlamadık.
Sonuç: Dikkatli bir cerrahi diseksiyonla, karotis cerrahisinin herhangi bir komplikasyona yol açmadan başarılı bir şekilde yapılabileceğini düşünmekteyiz.
Investigation of Cranial and Peripheral Nerve Injury During Carotid Endarterectomy
Objective: Injuries of cranial nerves such as vagus, hypoglossus and facial and peripheral nerves are sometimes reported in carotid surgery patients. In this study, we aimed to demonstrate the peripheral and cranial nerves injuries during carotid artery surgery in our patients with electrophysiological study which is an objective method.
Method: In total of 18 patients, electrophysiological evaluation of bilateral cranial and peripheral nerves was performed with standard fashion one [month] after the carotid endarterectomy + patch plastic operation. Also, in these patients and in 20 healthy control subjects, R-R interval variability during quiet and deep breathing was measured for evaluation of vagal nerve.
Findings: There were left carotid lesions in eleven patients (61%) and patch-plasty had been performed with polytetrafluoroethylene in fifteen patients and saphenous vein graft in three patients following carotid endarterectomy. Any major complication such as perioperative stroke and mortality was not observed in all of the patients. Preoperatively suspected bilateral carpal tunnel syndrome was confirmed with electrophysiologically in one patient. In postoperative period, partial facial nerve injury was detected in one patient (5,5%). Except this one, cranial and peripheral nerve injuries were not detected in remained patients who contain three patients with preoperative stroke remnant. There were not statistically significant differences in comparison of nerve conduction velocities and amplitudes of operation side and opposite side. There was not a statistically significant difference between the control group and carotid endarterectomy group in measurement of R-R interval variability during quiet and hyperventilation.
Discussion: Carotid endarterectomy operation is usually performed for prevention of stroke, but it can sometimes cause some undesirable complications. Injury of the branches of facial nerve, vagal nerve and hypoglossus are frequently reported. In majority of the studies, nerve lesion is demonstrated with investigation of suspected patients with complaints. We were not found a study which all of the carotid endarterectomy patients were evaluated with electrophysiologically. In our study which contain of electrophysiological investigation of some cranial and peripheral nerves including the vagal nerve, we were observed only one partial facial nerve lesion.
Conclusion: In extracranial carotid artery lesions, carotid andarterectomy operation can succesful-ly perform with carefully surgical dissection without any complication, although it can cause cranial and peripheral nerve complications which reported in literature.
