Dorsal penis siniri uyarımı ile elde edilen sakral kord ve kök potansiyelleri ve bulbokavernüz refleksin sakral kord içi iletimi

Dorsal penis siniri, tibial ve sural sinir uyarımları ile segmental spinal kord ve kök potansiyelleri epidural olarak normal deneklerden yazdınlmıştır. Aynı bipolar epidural iğne elektrodlar yolu ile sakral kord ve kökler uyarılarak bulbokavernöz (BC) kas ve çeşitli bacak kaslarından EMG yanıtları elde edilmiştir.
Penisten Tı2-Lı invertebral düzeye dek afferent iletim hızı ortalama 40 m/s bulundu. Bu iletim değeri alt ekstremite mikst ve deri sinirlerinin uyarımı ile elde edilenlerden anlamlı derecede daha yavaştır.
Maksimal epidural uyarılma ile T12-L1 intervertebral (İV) düzeyden BC kasına motor yanıtların latansı, daha kısa mesafeye karşın uyluk kaslarına olandan kıyaslanabilir derecede daha uzamış bulundu. BCR'nin sakral omurilik içindeki sentral iletim gecikmesi hesaplandı ve yaklaşık 8.2 msec bulundu. Aynı değer Soleus -H- Refleksi için 1.1 msec idi. Bu bulgular, daha hızlı oligosinaptik bağlantılar var olmakla beraber, BCR'nin ilk komponenti için gerekli 5-6 sinapsın var olabileceğini göstermektedir
Sacral cord and root potentials in response to stimulation of the dorsal nerve of penis and conduction of the bulbocavernosus reflex within the sacral cord
Segmental spinal cord and root potentials in response to stimulations of the dorsal nerve of penis, tibial nerve and the sural nerve were epidurally recorded in normal subjects. Same epidural needle electrodes were also for the bipolar stimulation of the sacral cord and the lumbosacral roots and the evoked EMG responses were obtained from the Bulbocavemosus (BC) and from the various leg muscles.
The afferent conduction velocity from the penis to T-12 Lj intervertebral level was about 40 m/s on the average which is significantly slower than those obtained by the stimulation of the mixed and cutaneous nerves at the lower limb. The latency of the motor responces of the BC muscle from T-i2-Li spine levels were fond comparatively longer than those of thigh muscles on maximal epidural stimulation in spite of the shorter distances to the BC muscle. The central conduction delay within the sacral cord of the Bulbocavemosus Reflex was calculated and found to be about 8.2 msec, while the central conduction time was about 1.1 msec, for the Soleus-H-Reflex. These findings suggest that there may be about 5-6 synapses necessary for the first component of the Bulbocavemosus reflex though some faster oligosynaptic cord linkage may also exist.
