Lumbosakral ladikülopatinin tanısında dermatomal semotosensoryal evoked potansiyeller

Bu çalışmanın amacı dermatomal somatosensoryal evoked potansiyellerinin (DSEP), lumbosakral radikülopati tanısında kullanılıp, kullanılmayacağını araştırmaktır. Daha önceki çalışmaların sonuçlan çelişkilidir. Kırk beş hasta DSEP, elektromiyografi (EMG), iumbosakral bilgisayarlı tomografi (BT) ve/veya magnetik rezonans görüntüleme (MRG) ile incelendiler. DSEP, EMG, BT/MRG sonuçlan operasyon bulgulan ile karşılaştırıldığında, DSEP olgulann % 53'ünde lezyon düzeyini tam olarak gösterirken, EMG ve BT/MRG olgulann % 80 ve 83'ünde tam lokalizasyon yapülar. Lumbosakral radikülopati tanısında, DSEP'lerin, EMG ve görüntüleme tetkiklerine göre daha az duyarlı ve güvenilir olduğu ve adı geçen tanı yöntemleri ile birlikte kullanılırsa yararlı olabileceği sonucuna varıldı.
The use of dermatomal somatosensorial evoked potentials in diagnosis of lumbosacral radiculopaties
Aim of this study is whether the dermatomal somatosensory evoked potentials (DSEP) may be used in the diagnosis of lumbar radiculopathies. We evaluated 45 patients with lumbosacral radiculopathy by DSEPs, conventional electromyography (EMG), lumbosacral computerized tomography (CT), and/or magnetic resonans imaging (MRI). Commparison was made among the results of DSEPs, EMG, CT/MRI and operation findings. When DSEPs were compared with operation findings, DSEPs made the correct localization in approximately half of the cases. EMG and CT/MRI findings made a good correlation with those of operations in approximately 80-83 % of cases respectively. In several cases, it is concluded that DSEPs are less sensitive and accurate than EMG and imaging techniques. DSEPs may be used in conjunction with EMG and imaging techniques in diagnosis of lumbosacral radiculopathies.
