Bu yazıda, posthipoksik myoklonus (Lance-Adams Sendromu) tanısıyla izlenen bir olgu sunulmaktadır. 50 yaşında kadın hastada tiroidektomi operasyonu komplikasyonu sonucu gelişen hipoksi sonrası ilk 24 saatte koma ve jeneralize myokloniler gelişmiştir. Bilinç açıldıktan sonra da devam eden, istemli hareketle aktive olan yaygın myokloniler nedeniyle EEG, kranyal BT, MRG, SPECT, somatosensoriyel ve beyin sapı işitsel uyarılmış potansiyel incelemeleri yapılmış ancak bu incelemeler konunun fizyopatolojisi hakkında aydınlatıcı olmamıştır. Olgumuzda 6 mg. klonazepam ve 2000 mg. Na-valproat kombine tedavisiyle belirgin düzelme görülmüştür.

In this study, a case with posthypoxic myoclonus (Lance-Adams Syndrome) is presented. A 50 [year] old woman who had prolonged hypoxia as the complication of a thyroidectomy operation, remained comatose and then development generalized myoclonic jerks in the first 24 hours. After she gained consciousness, the myoclonic jerks persisted which were more exagerated with voluntary movements or emotional changes. EEG, CT, MRI, SPECT, somatosensory and barinstem auditory evoked potential studies were not yielding additional information of the physiopathologic process. Remarkable improvement was noted with the combined therapy of 6 mg. clonazepam and 2000 mg. Na-valproate.
