Tepkisel Bağlanma Bozukluğu (TBB) çocuklarda 5 yaşından önce başlayan, genellikle sağlıksız koşullarda (temel bakım verme koşullarında, çocuğun rahatlatılması, uyarılması ve sevgi verilmesi gibi temel duygusal gereksinimlerin göz ardı edildiği koşullar, çocuğun temel fizik gereksinimlerinin sürekli görmezlikten gelindiği koşullar veya kalıcı bir bağlanmanın oluşmasını önler biçimde birincil bakım verenin sık sık değiştiği koşullar) çocuk yetiştirmeye bağlı olarak gelişen, ikili ilişki kurmada ve başka toplumsal etkinliklerde, yaşına uygun olmayacak şekilde önemli bozuklukların ortaya çıkmasıdır (DSM IV TR). Bu yazıda, TBB tanısı alan üç vak’a sunulmuş olup, ebeveynlerinde ruhsal sıkıntı öyküsünün eşlik ettiği sağlıksız bakım vermenin, özellikle gelişimsel yatkınlığı bulunan çocuklarda, bu tabloya yol açtığı ve de bu vak’aların, başvuru döneminde, otizm benzeri belirtiler de göstermiş oldukları halde, tedavi amaçlı olarak aldıkları özel eğitim ve âile danışmanlığı süreci sonucundaki klinik seyirlerinde, söz konusu belirtilerin tamamının düzelmiş olması yâni çocukların sosyal ilişkilerinin, dil ve öğrenme yeteneklerinin, bilişsel ve motor işlevlerinin tamamen normâl gelişimsel standartlara ulaşması tartışılmıştır. Bu sonuçlar TBB’nin tedaviye alındıktan sonra düzelme gösterdiğine dâir bilgilerle uyumludur.

The diagnosis “Reactive Attachment Disorder” refers mainly to a disturbance in the childparent attachment relationship. The abnormal development of social behaviors in children, occurs, generally in response to pathological caregiving such as, either “persistent disregard of the child’s basic emotional needs for comfort, stimulation and affection”, “persistent disregard of the child’s basic physical needs” or repeated changes of primary caregiver that prevent formation of stable attachment”. These disturbances in social behavior start before the age of five (DSM IV TR). Social dysfunction is the main feature of children with “Reactive Attachment Disorder”. In the inhibited subtype, children, persistently fail to initiate and respond to social interactions in an age appropriate manner. They generally show ambivalence in response to social interactions. They are usually miserable and show resistance to comforting efforts. In the disinhibited subtype, children do not make selective attachments and their sociability is indiscriminated. They may exhibit aggressiveness and they may have problems with their peer relationships. Communication problems observed in children with “Reactive Attachment Disorder” tend to be more similar with the symptoms seen in [language] disordered children and less similar with the problems seen in autistic children. Prognosis depends largely on the nature, duration and severity of pathogenic care experienced by the child and on the child’s own characteristic features. Generally, severity and duration of exposition to pathological caregiving determine the prognosis. Three “Reactive Attachment Disorder” patients between ages 1.5 to 2.5, whose parents came up with pathological caregiving due to depression, are presented in this paper. Although, these patients had autism like symptoms in the beginning, after their psychoeducational treatment program, special management and family counseling sessions, all of their initial symptoms, showed dramatically improvement. Their social interactions and peer relationships, linguistic and learning abilities, cognitive functioning and motor development reached age appropriate level. This finding is consistent with the knowledge that, symptoms of Reactive Attachment Disorder improve after intervention.
