Nörosifiliz, sifilizin Santral Sinir Sistemi (SSS) invazyonu ve nörolojik belirtilerle seyreden bir formudur. Demans, depresyon ve grandiozite nörosifilizin en sık görülen psikiyatrik belirtileridir. Nörosifilizin oluşturduğu psikiyatrik tablolar, fonksiyonel psikiyatrik hastalıklara büyük benzerlik gösterir veya birçok psikiyatrik hastalığı taklit edebilir. Nörosifiliz psikiyatrik hastalığa sebep olmaktan farklı olarak psikiyatrik hastalığın prognozunu da etkileyebilir. Şizofrenideki psikotik epizodların sıklığı artabilir, tedaviye cevap zorlaşabilir ve bilinçte bulanıklığa sebep olabilir. Tedavinin izlenmesi için de kullanılan non-treponemal testler arasında BOS'ta (Beyin Omurilik Sıvısı) VDRL seviyesini ölçmek tanı konması açısından değerlidir ancak bu sonucun daha duyarlı olan treponemal testlerle doğrulanması gerekir. Nörosifilizde en etkin tedavi yüksek doz i.v. kristalize penisilin'dir. Burada 7 ay önce âni olarak başlayan uykusuzluk, küfürlü ve kendi kendine konuşmaları, garip davranışları olan ve aldığı antipsikotik tedaviye rağmen psikotik belirtilerinde düzelme olmayan sonradan yapılan tetkiklerinde nörosifiliz tanısı alan bir hastayı sunduk.
Nörosifilizin birçok psikiyatrik hastalığa benzer görünüm gösterebilmesi, tanı karmaşası yaratması bu hastalığı gizemli bir hâle getirmektedir. Bu olguyu sunmaktaki amacımız nörosifiliz hastalarının salt nöropsikiyatrik belirtilerle başvurabileceklerini belirtmek ve özellikle dirençli psikotik bozuklukta bu hastalığı akla getirmek gerektiğine dikkati çekmektir.

Neurosyphilis is a form of syphilis with invasion of CNS (Central Nervous System) and neurological signs. Dementia, depression and grandiosity are commonly observed psychotic signs in neurosyphilis. Psychiatric symptoms due to neurosyphilis resemble functional psychiatric diseases or they can mimic psychiatric diseases. Neurosyphilis can effect the prognosis of the psychiatric disease instead of causing the illness. Frequency of psychotic episodes in schizophrenia may increase, response to treatment can decrease and organic involvement can cloud consciousness. Monitoring VDRL titers in CSF (Cerebrospinal Fluid) is valuable for the diagnosis but it has to be confirmed with non-treponemal tests. Effective treatment of neurosyphilis is iv form aqueous penicillin. We present a 42- [year] old male patient with symptoms of insomnia, speaking by oneself and cursing speech, strange behaviors. There was no improvement on his psychotic symptoms even though application of antipsychotic medication. His serum and CSF detection findings showed the diagnosis of neurosyphilis.
Neurosyphilis is a mysterious disease because of imitating psychiatric diseases and leading difficulties in differentiating the diagnosis. Our aim of presenting this case is that neurosyphilis is presented with only neuropsychiatric symptoms and especially in resistant psychotic disorder neurosyphilis should be evaluated in differentiating the diagnosis.
