Obsesif Kompulsif Bozukluk, Serotonerjik Sistem ve Serotonin Gerialım İnhibitörleri (SSRI)

Potent serotonin geri alım blokerleri olan klomipramin, şuvoksamin ve şuoksetinin obsesifkompulsif bozukluğun (OKB) tedavisinde de etkin olduklarının gözlemi serotonerjik sistemdeki bir işlev bozukluğunun OKB’un fizyopatolojisi ile ilişkli olabildiğinin öne sürülmesine yol açmıştır. Beyinomurilik sıvısı (BOS) 5-hidroksindolasetik asid (5-H1AA) ve trombosit serotonin (5-HT) düzeyleri nöroendokrin (5-HT agonistlerine [örneğin, MK-212, m-klorofenilpiperazin] endokrin yanıtların azalması) .ve trombosit imipramin bağlanma çalışmaları OKB'ta serotonerjik iletimin rolüne ilişkin kanıtları sağlamıştır. Bu makalede OKB tamlı hastalarda tek başına veya selektif serotonin geri alım inhibitörleri ile kombine bir şekilde serotonerjik etkili diğer ilaçların kullanıldığı araştırmalar kısaca gözden geçirilmiş ve değerlendirilmiştir.
Obsessive Compulsive Disorder, Serotonergic System and Serotonin Reuptake Inhibitors
The observation tht clomipramin, suvoksamin, and suoxetine, which are potent blockers of serotonin (5-hydroxytryptamin [5-HT] uptake, are also effective in the treatment of obsessive-compulsive disorder (OCD) has led to the hypothesis that a dysfunction in the serotonergic system may be associated with the pathophysiology of OCD. Evidence related toh the role of SAT neurotransmission in OCD has been divided by the studies of cerebrospinal suid (CSF) levels of 5-hydroxy-indolacetic acid (5-HIAA), platelet 5-HT levels, and neuroendocrine (decreased endocine responses to 5-HT agonists [e.g, MK-212, m-chlorophenylpiperazine] and platelet imipramine binding studies. In this article is reviewed briesy and evaluated in the context of investigations that used other drgus with serotonergic actions either alone or in combination with selective re-uptake inhibitors (SRIS)in OCD.
