Erken Başlangıçlı Şizofreni Hastalarında “Döner Kapı Fenomeni”: Çok Merkezli Bir Çalışma

Amaç: Bu çalışmada, erken başlangıçlı şizofreni (EBŞ) tanılı olguların Döner Kapı Fenomeni (RDP) kapsamında değerlendirilmesi amaçlanmıştır.
Gereç ve Yöntem: Türkiye’de üç merkeze ait çocuk ve ergen psikiyatrisi yataklı servis verileri geriye dönük olarak incelenerek; olguların
sosyo-demografik özellikleri, psikiyatrik eş tanıları, tedavi modaliteleri, tedavi öncesi ve sonrası değerlendirmeleri uygulanan Pozitif ve Negatif
Sendrom Ölçeği (PANSS) ve Çocuklar için Global Değerlendirme Ölçeği (CGAS) puanları ile değerlendirilmiştir. Çalışmaya dahil edildiği yatışla
beraber 18-60 ay takip edilen olgular içinde tekrarlayıcı olan hastalar değerlendirilerek, iki ve üzeri yatışı olan hastalar RDP (+) olarak kabul
edilmiştir.
Bulgular: RDP (+) (n=40) ve RDP (-) (n=35) grupları arasında sosyodemografik özellikler açısından sadece madde kullanım bozukluğu
öyküsü (p=0,007) karşılaştırıldığında anlamlı fark saptandı. İki grup arasında yatışları sırasında kullanılan psikotrop ilaçlar karşılaştırıldığında
haloperidol (p=0,001) ve Birinci Kuşak Antipsikotikler (BKA) (p=0,011) açısından farklılıklar saptanmıştır. Sonuçlar, toplam PANSS yatış
(p=0,044) ve CGAS yatış skorları (p=0,006) için tanımlanan 18-60 ay takibi olan hastaların yatış sayısı arasında anlamlı bir etkileşim olduğunu
saptadı. Yapılan regresyon analizlerinde madde kullanım bozukluğu öyküsü, BKA kullanımı ve yatış toplam PANSS puanlarının EBŞ tanılı
olgularda RDP için anlamlı düzeyde yordayıcı faktörler olduğu bulunmuştur.
Sonuç: Çalışmamızda, belirli semptomatolojik boyutların erken tanımlanmasına, madde kullanım bozukluğu öyküsünün değerlendirilmesi ve
olguların tedavi yönetimine odaklanan hedefli stratejilerin RDP’yi azaltmaya yardımcı olabileceğini düşündürmektedir.
The “Revolving Door Phenomenon” in Patients with Early Onset Schizophrenia: A Multicenter Study
Objectives: The aim of the study was the evaluation the data of patients with early onset schizophrenia (EOS) within the scope of the
Revolving Door Phenomenon (RDP).
Materials and Methods: Data obtained from three child and adolescent psychiatry inpatient services in Turkey were retrospectively
evaluated and sociodemographic characteristics, psychiatric comorbidities, treatment modalities, pre- and post-treatment assessments of the
patients were assessed with the Positive and Negative Syndrome Scale (PANSS) and Children’s Global Assessment Scale (CGAS). Patients with
more than one hospitalization within 18-60 months of follow-up from the admission to the study were evaluated, and patients with two or
more hospitalizations were accepted as RDP (+).
Results: Between RDP (+) (n=40) and RDP (-) (n=35) groups, a significant difference was found when only history of substance use disorder
(p=0.007) was compared in terms of sociodemographic variables. When the psychotropic drugs used during their hospitalization were compared
between the two groups, differences were found in terms of haloperidol (p=0.001) and First Generation Antipsychotics (FGA) (p=0.011). The
findings showed a significant interaction between the number of hospitalizations of patients with 18-60-month follow-up defined for total
PANSS hospitalization scores (p= 0.044) and CGAS scores (p=0.006). In the regression analyses, it was found that the history of substance use
disorder, FGA use and total PANSS scores of hospitalization were significant predictive factors for RDP in patients with EOS.
Conclusion: In our study, findings suggest that personalized strategies focusing on early identification of clinical dimensions, evaluation of
substance use disorder history, and treatment management of cases may help reduce the effects of RDP.
