Ergenlik Dönemin Psikotik Çöküş Üzerine Bir Vaka Çalışması

Birçok psikanalist, ergenlik döneminde; bedensel değişimler, kimlikle ilgili sorgulamalar ve ödipal bağların canlanması nedeniyle psikotik çöküşler meydana gelebileceğini söylemektedir. Bu psikanalistlerden Laufer, psikotik çöküşün her zaman kalıcı olmayabileceğini söylerken; bir ergenin psikoz tanısı alması için belli kriterler öne sürmektedir. Annesini öldüreceği korkusuyla psikiyatriye başvuran psikiyatrist tarafından psikoz olabileceği şüphesiyle yönlendirilen 17 yaşındaki hastaya psikanalitik ön görüşme ile birlikte Rorschach uygulaması yapılmıştır. Hasta, 17 yaşında, bir Anadolu Lisesi'nin 11. sınıfında okuyan, akademik düzeyi iyi bir gençtir. Annesi ile yakın bir ilişkisi olduğunu söyleyen hasta babasıyla ilişkisinin mesafeli olduğunu söylemektedir. Ön görüşmesine dair önemli görülen bilgilerden biri hastanın 10 yaşında Ankara'dan İstanbul'a göç etmesidir. Hasta, bunun kendisi için zor bir deneyim olduğunu söylemektedir. Test içeriği incelendiğinde ise, hastanın ayrılmak konusunda sıkıntı yaşadığı ve annesiyle simbiyotik tarzda bir ilişkisi olduğu bulgulanmıştır. Bu çalışmanın amacı, ergenlik döneminde yaşanan psikotik çöküşün Rorschach Testi ile incelenmesi olarak belirlenmiştir.
A Case Study on Psychotic Breakdown in Adolescence

Unable to comprehend the integrity of the body and vaguenes of the borders are the foremost and striking matters in the test with the applied Rorschach Test, borderline disorder and narcissistic defences are observed. Reflection of splitting on the perseption of body as seen commonly at borderline disorder forms the axis of this study.
