Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) yaşamı bütün dönemlerinde etkileyen yaygın bir psikiyatrik bozukluktur. DEHB’nin şiddeti ve beraberindeki eş tanılar hangi rahatsızlığın önce tedavi edilmesi gerektiğine de yol gösterir. DEHB ile Bipolar Bozuklukların (BB) hem bir arada görülmesi hem de birbirinden ayırt edilme gereksinimi uzun zamandan beri tartışılmaktadır. DEHB ve BB’nin her ikisi de çelinebilirlik, aşırı konuşma, duygudurum oynamaları, agresyon ve kronik irritabilite belirtilerini sergiler. Bu iki bozukluk arasındaki belirti örtüşmesi hem ayırıcı tanı sorunlarına hem de tedavi süreçlerinin doğru yönetilmesi ile ilgili güçlüklere yol açmaktadır. Belirti bazında ayırıcı tanı kolay olmamaktadır. Tanısal karmaşayı azaltmak, daha doğrusu DEHB ile BB arasındaki farklılıkları daha iyi ortaya koyabilmek için (daha konservatif bir tanımla) dönemselliğin ve tanı belirginliğinin yüksek olduğu BB-Tip I ve BB-Tip II’yi odağa almak ve DSM ölçütleri ile hareket etmek yerinde olacaktır. BB hastalarında DEHB tanısı da değerlendirildiğinde belirtileri ayırt etmenin en geçerli yolu kapsamlı bir klinik değerlendirmeyle yapılabilir. İki hastalığı en iyi şekilde ayıran temel bulgu hastalıkların seyri ile ilgilidir. BB dönemsel seyirli iken, DEHB süreğen gidişlidir. Belirli bir süre boyunca uykuya duyulan ihtiyacın azalması, psikotik belirtiler ve artan benlik saygısı gibi belirli BB belirtilerinin varlığı, iki hastalık arasında ayrım yapılmasına yardım edebilir. DEHB tanılandırmasında öz bildirim ölçeklerini temel alarak fazla tanı konması, DEHB tanısının az konması kadar kötüdür. Çünkü BB hastalarında yersiz DEHB tedavisi uygulanması duygudurumun düzenlenmesini olumsuz etkileyebilir.

Attention deficit hyperactivity disorder (ADHD) is a common psychiatric disorder affecting all stages of life. The severity of ADHD and associated co-morbid disorders also guide us towards which disorder to treat first. There is a longstanding debate about the coexistence of ADHD and bipolar disorders (BD) and the need to differentiate both from each other. ADHD and BD both include symptoms of distractibility, talkativeness, emotional lability, aggression and chronic irritability. The symptomatic overlap between these two disorders leads to problems in the differential diagnosis as well as difficulties in the proper management of the treatment. In the context of symptomatology the differential diagnosis is not easy. To decrease the diagnostic difficulties, more accurately to better reveal the differences between the ADHD and BD (a more conservative definition), it would be appropriate to focus on the BD-type I and BD-type II where the episodicity and diagnostic clarity are obvious and to move according to DSM criterion. When the diagnosis of ADHD is considered in BD patients, the best way to differentiate the symptoms is via a clinical evaluation. The main sign differentiating the both diseases is the one involving the course of illness. BD is episodic whereas ADHD is consistent over time and across settings. The presence of specific BD symptoms, such as reduced need for sleep for a selected period, psychotic symptoms, and increased self-esteem may help to discriminate between the two entities. Over-diagnosis based on the self-report questionnaires is as bad as under-diagnosis, since the treatment of ADHD has unfavorable consequences on mood stability among BD patients.
