Toplum temelli ruh sağlığı anlayışının en temel kurumları Toplum Ruh Sağlığı Merkezleri’dir (TRSM). TRSM’lerde bakım hizmetlerinin planlanması ve koordinasyonu, sürecin sağlıklı işlemesi ve belirlenen bakım hedeflerine ulaşma açısından büyük önem taşımaktadır. Toplum temelli bakım hizmetlerinin sunumu için gereken mekanizma, birden çok akademik disiplinin bir araya geldiği bir ekip oluşumunu gerektirir. TRSM’lerde bakım planlarının yapılması, uygulanması ve takip edilmesi vaka yöneticileri tarafından sağlanmaktadır. Bakım hizmetlerinin koordinasyonuna duyulan ihtiyaç, ilk olarak ABD’de, ardından da Avrupa ve İskandinav ülkelerinde 1960’lar ve 1970’lerde kurumsal sağlık hizmetlerinden toplum temelli bakım hizmetlerine yönelimin başlamasıyla belirgin hale gelmiştir. Bakım hizmetlerinin koordinasyonu sayesinde, hem doğrudan hizmetin kendisi sunulmuş, hem de karmaşık ve dağınık durumdaki kurumsal sistemlerde verilen hizmetler koordine edilmiştir. Bakım hizmetlerinin koordinasyonu, hizmet sistemi ile hizmet faydalanıcısını bir araya getirmeye ve olumlu sonuç elde etmek için çeşitli sistem parçalarını koordine etmeye yönelik yürütülen faaliyetleri kapsar. Geleneksel yaklaşım soruna odaklanır. Bireyin güçlü yanları genellikle değerlendirilmez ya da sorunları ele alırken göz önünde bulundurulmaz. Sorunlarıyla başa çıkabilmek için bireye yardımcı olan unsurlar, bireyin güçlü yanlarıdır. Yardım süreci hastaların eksiklerine, zayıf yönlerine veya problemlerine değil, güçlü yönlerine, ilgi alanlarına, becerilerine ve kapasitelerine odaklanır. Bu süreçte yapılan değerlendirme, faydalanıcının hayat manzarasını bütüncül bir şekilde resmetmelidir. Değerlendirmenin ortaya çıkardığı somut sonuç “bakım planı”dır. Planın işlevi, hizmet faydalanıcısının ihtiyaçları ve güçlü yönleri ile bireyin içinde bulunduğu durumun iyileştirilmesi için atılacak adımları kaydetmektir. Bakım planı, değerlendirme ve sonuçların sürekli bir şekilde izlenmesiyle birlikte değişiklik gösteren, dinamik ve devam eden bir kayıt sürecidir. Süreç esnek ve süreklidir. TRSM yerel bakım koordinasyon ortaklarının tespit edilmesi gereklidir. Vaka yöneticisi tarafından, bakım planı temel alınarak, bu resmi ve sivil kaynaklar ile temasa geçilerek bakım hedeflerine ulaşmaya çalışılır.

Community Mental Health Centers (CMHC) are the most basic institutions of community based mental health care. The planning and coordination of care services in CMHCs is of great importance for the healthy functioning of the process and for achieving the determined care targets. The mechanism required for the provision of community-based care services requires the formation of a team of multiple academic disciplines. In community mental health centers, case managers plan, implement and monitor care plans. The need for coordination of care services was first apparent in the United States and then in Europe and Scandinavian countries in the 1960s and 1970s with the introduction of community based care services from institutional health services. Thanks to the coordination of care services, both the direct service itself is presented and the services provided in complex and disorganized corporate systems are coordinated. Co-ordination of care services includes activities carried out to bring the service system and service beneficiary together, and to coordinate various system components to achieve positive results. The traditional approach focuses on the problem. The strengths of the individual are usually not considered or taken into account when addressing problems. The elements that help the individual to cope with their problems are the strengths of the individual. The help process focuses on the strengths, interests, skills and capacities of the patients, not on their shortcomings, weaknesses or problems. The assessment made in this process should illustrate the life scene of the beneficiary in a holistic manner. “Care plan” will be the concrete result of the evaluation. The function of the plan is to record the steps to be taken to improve the situation of the individual with the needs and strengths of the service beneficiary. The care plan is a dynamic and ongoing recording process that changes with evaluation and continuous monitoring of results. The process is flexible and continuous. Community mental health center local care coordination partners need to be identified. The case manager tries to reach the maintenance targets by contacting these official and civil resources based on the care plan.
