Son iki yüzyılda kaynakları iyi olan ülkelerdeki ruh sağlığı hizmetlerinin evrilmesine bakacak olursak, her ülke için başlangıç ve bitiş tarihleri değişmekle birlikte, kabaca üç döneme ayırabiliriz: Akıl hastanelerinin yükselişi, akıl hastanelerinin düşüşü ve toplum temelli ruh sağlığı hizmetlerinin gelişmesi. Toplum temelli hizmetlerin gelişmesi sürecinde ruh sağlığı hastanelerinin yerini küçük birimler almış, toplum içinde yerleşik multidisipliner ekipler oluşturulmuş ve kanıta dayalı psikiyatri uygulamaları ön plana çıkmıştır. Uzun dönem yatışların yapıldığı bölge hastanelerinin yatakları azaltılmış ya da adli yataklara çevrilmiş, toplum ruh sağlığı ekiplerinin sayıları artırılmıştır. Yaşam kalitesinin artırılması ve relapsın önlenmesi konuları ön plana çıkmıştır. Türkiye’de toplum temelli ruh sağlığı hizmetlerinin geliştirilmesine yönelik çalışmalar 1960’lı yıllarda başlamış ancak yaygınlaştırma ve sürdürülebilirlikle ilgili sorunlar nedeni ile sınırlı kalmış veya sonlandırılmak zorunda kalmıştır. 2006 yılında toplum temelli ruh sağlığı hizmet modelinin ülke politikası olarak kabul edilmesi gerçekleşmiş, Sağlık Bakanlığı tarafından bu alanla ilgili istenilen modern vurguları esas alan “Ulusal Ruh Sağlığı Politikası” metni yayınlanmıştır. Toplum temelli ruh sağlığı uygulamalarını güçlendiren bir diğer adım 2011 yılında yayınlanan ilk Ulusal Ruh Sağlığı Eylem Planı’nın ana hedefinin toplum temelli ruh sağlığı hizmet modeline geçiş olarak belirlenmesi olmuştur. İlk olarak Bolu’da hizmet vermeye başlayan Toplum Ruh Sağlığı Merkezleri (TRSM) hızla ülke geneline yaygınlaştırılmış, barınma, sosyal bakım ve istihdam alanlarında yapılan düzenlemeler ile toplum ruh sağlığı hizmetleri desteklenmeye çalışılmıştır. Sonuç olarak koordinasyon eksiklikleri, eğitim ve kaynak aktarımı başta olmak üzere sürdürülebilirliğe dair sorunlar olsa da Türkiye’de toplum temelli ruh sağlığı alanında son yıllarda yaşanan gelişmeler umut vericidir.

If we look at the evolution of mental health services in countries with good resources in the last two centuries, we may roughly define three periods for each country, with differing starting and ending periods: the rise of mental hospitals, the decline of mental hospitals and the development of community-based mental health services. In the process of development of community based services, mental health hospitals were replaced by small units, multidisciplinary teams were established in the community and evidence-based psychiatry practices become prominent. Beds of regional hospitals for long-term hospitalizations were either reduced in number or converted into forensic beds and the number of community mental health teams have been increased. Improving quality of life and relapse prevention issues have come forward. Work for developing community-based mental health services in Turkey began in 1960’s, but remained limited or had to be terminated due to problems related with dissemination and sustainability. In 2006, community-based mental health service model was accepted as the country policy and National Mental Health Policy was published by the Ministry of Health based on the desired modern emphasis on this field. A further step in strengthening community-based mental health practices was the identification of transition to a community-based mental health service model as the main target of the first National Mental Health Action Plan published in 2011. Community mental health centers which first started to serve in Bolu, were rapidly disseminated throughout the country, and these services were supported with arrangements in areas of housing, social care and employment. As a result, developments in communitybased mental health field in Turkey are promising in recent years, despite the problems about sustainability including coordination defects, education and funds transfer.
