Manik dönem bipolar bozukluğun seyrinde görece daha az yer tutsa bile, belirtilerin gürültülü olması ve hastanın ve çevresinin üzerinde daha fazla etki oluşturması nedeniyle dikkat çekicidir. Manik dönemin tedavisinde tüm antipsikotikler etkilidir ve başarıyla kullanılmaktadır ancak yine de tedavisi zor olan olgular ile gündelik rutin uygulamada karşı karşıya kalınmaktadır. Bu yazıda şiddetli ve tedaviye tam yanıt vermeyen bir olgu, duygudurum belirtileri düzelse bile psikotik belirtileri kısmen süren bir olgu, manik dönem belirtileri kronikleşen ve yaşamına egemen duruma gelen bir olgu ile yaratıcılığı açısından manik dönemi tercih eden ve sürmesini isteyen bir ressam olgu sunulmuştur ve tartışılmıştır.

Even though manic episode has a relatively narrow place in the course of bipolar disorder, since the symptoms are remarkable and cause more effects on the patient’s and significant others’ life, it is prominent. All antipsychotics are effective and being successfully used in the treatment of manic episode, but difficult-to-treat patients are ought to be dealt with in the routine daily practice. In this article, one case with severe mania not responding to treatment, one case with psychotic residual symptoms even though mood symptoms are remitted, one case with chronic manic symptoms predominating his life, and one case who is a painter and who prefers to be manic for her creativity are presented and discussed.
