Ailesinde șiddet gören kadına poliklinikte yaklașım: Kullanıșlı bir yöntem önerisi olarak destekleyici psikoterapi

Aile içi şiddet, ailenin değişik bireyleri arasında ortaya çıkabilen değişik şiddet tiplerini içeren geniş bir kavramdır. Bu makalenin kapsamına giren
bireyler, bundan payını en çok alan yetişkin aile üyeleri olarak kadınlardır. Toplumumuzdaki ve dünyadaki yaygınlığı ve olumsuz sonuçları nedeniyle, aile içinde kadına yönelik şiddet gözardı edilmemesi gereken bir sorundur. Yapılan çalışmalar, Türkiye’de en iyi olasılıkla her üç kadından
birisinin bu sorunla karşı karşıya gelmekte olduğunu göstermektedir. Psikiyatri kliniklerinde bu oran çok daha fazladır. Öte yandan, akut fiziksel
şiddet durumlarında çoğu zaman ilk başvurulan yer psikiyatri değildir. Ruh sağlığı alanında çalışan bizler, “Şiddet görüyorum”diyerek gelen hastadan daha çok depresyon, anksiyete bozukluğu veya somatik yakınmalar ile gelen hastalarda, ayrıntılı bir sorgulama sonucunda fiziksel şiddet başta
olmak üzere hastanın şiddet deneyimi ile ilgili bilgi sahibi oluyoruz. Böylesi durumlar, ilaç dışında girişimlere gereksinim duyulan durumlardır.
Destekleyici psikoterapi ise, hedefi belirtileri iyileştirmek, benlik değerini ve ben işlevini desteklemek, uyum becerilerini onarmak, sürdürmek ve
geliştirmek olan dinamik bir psikoterapi türüdür. Esnek ve hemen hemen her hasta tipine uygulanabilecek teknikler üzerinden işler. Bu özellikleriyle, ailesinde şiddet gören kadınlara poliklinik ortamında uygulanabilecek bir seçenektir. Bu yazıda, bu bir öneri olarak sunulmaktadır. Önce
aile içi şiddet hakkında kısaca bilgi verilmiş ve destekleyici psikoterapi kısaca tanıtılmış; sonrasında bu öneriyi açıklamak üzere kuramsal bilgiler
eşliğinde bir hastanın psikoterapi sürecinde destekleyici tekniklerin kullanımından örnekler ayrıntılarıyla verilmiştir.
Outpatient approach to women subject to domestic violence: proposal of supportive psychotherapy as a useful method
Domestic violence is a broad concept containing various types of violence between various family members. Since they are the most affected
adult individuals, woman members are the main subject of this article. Due to its commonality in the world and in our country, and negative
consequences, domestic violence targeting women is a serious problem. Studies show that one in three women has been subject to domestic
violence in Turkey. This ratio is much higher among who are admitted to psychiatric clinics. On the other hand, in the case of acute physical
violence, psychiatry is usually not the first preferred place of admission. However, mental health workers frequently encounter with direct or
indirect consequences of domestic violence. Information about violence is obtained only if a detailed interview is conducted with a patient
who has somatic symptoms or symptoms of depression, anxiety disorder, etc. rather than hearing direct complaint about violence. Such
instances require interventions other than medication. Supportive psychotherapy is dynamic type of psychotherapy that aims to improve
symptoms and self-esteem, to support the I (ego) function, to repair, maintain and further develop adaptive skills. We propose supportive
psychotherapy as a useful tool for the management and treatment of women suffering from domestic violence and to help them break
the vicious cycle since supportive psychotherapy works through flexible and widely applicable techniques to almost all patient types and
in outpatient clinic environment. This proposition is derived from the synthesis of our literature search, our previous research findings
and our clinical observations and experience. In this article, some theoretical information is given about domestic violence and supportive
psychotherapy. Then the psychotherapy process of an “ordinary” outpatient case of chronic depression, a woman who has actually been
living in a life-long domestic violence environment(s), is discussed as an example to support our proposition. The use of various supportive
techniques in the psychotherapy process is demonstrated via clinical vignettes.
