Kanserli hastaya ve yakınına poliklinikte psikoterapötik yaklașım

Ölüm, ağrı ve acı çekme ile eş anlamlı olarak düşünülen kanser, hastaların hızla ele alınması gereken başa çıkması zor bir dönem geçirmelerini
tetikler. Kanser tanısı almak ve kanser tedavileri majör yaşam stresörleridir. Kansere psikolojik, fiziksel, sosyal, cinsel, maneviyat ve gündelik
uygulamalara yönelik pek çok sorun eşlik edebilir. Günümüzde erken tanı ve sağaltım olanaklarının gelişmesiyle beraber pek çok kanser hastası uzun yıllar yaşamaktadır. Kanser, kronik bir hastalık gibi yönetilebilir. Kronik hastalık olarak kanser bakımı, önlemeden yaşamın son günlerine değin uzanan bir süreci kapsar. Kanser tanısı alan hastalar bu duruma psikolojik tepkiler göstererek uyumlarını yaparlar. Kanserli hasta
ve yakınlarıyla psikoterapötik ilişki ilk görüşmeden itibaren kurulmaya başlar. Poliklinik koşullarında kanserli hasta ve yakınına psikoterapötik
yaklaşımda empatik ve destekleyici görüşmeler esastır. Kanser hastalarıyla çalışırken pek çok klasik terapi yöntemi bu hastalığa göre yeniden
yapılandırılmış, ayrı terapi teknikleri harmanlanarak yeni terapiler geliştirilmiştir. Kanserde sıklıkla kullanılan terapiler içerisinde destekleyici
psikoterapi, bilişsel davranışçı terapi, kısa dinamik psikoterapi, destekleyici dışavurumcu psikoterapi, bilişsel analitik psikoterapi, farkındalık
terapisi, anlam odaklı terapi, gevşeme ve imgelem temelli terapi, motivasyonel görüşmeler-danışmanlık, saygınlık terapisi sayılabilir. Kanser
hastaları tanı öncesi tetkik, tanının söylenmesi, akut sağaltım, remisyon, tekrarlama-metastaz, terminal-palyatif dönemlerde çeşitli ve değişik
duygusal, ruhsal, davranışsal tepkiler geliştirirler. Bu tepkilerin bir kısmı normaldir ve uyuma yöneliktir. Bu makalede kanserli hasta ve yakınlarına psikoterapötik yaklaşım örneklerle açıklanacaktır.
Psychotherapeutic approach to cancer patients and their relatives at outpatient unit
Cancer diagnosis which is used as synonym of “death”, “pain” and “sorrow”leads to a hard-coping period in these patients which requires a
rapid intervention. The diagnosis and subsequent treatment of cancer is a major life stress. Cancer is associated with psychological, social,
physical, sexual, spiritual and practical difficulties. With recent improvements in early detection, diagnosis and treatment of cancer, people
with cancer are living longer, and their cancer may be managed as a chronic illness. As a chronic illness, cancer care occurs on a continuum
that stretches from prevention to the end of life. Patients diagnosed with cancer make their adjustment after eliciting psychological responses.
Psychotherapeutic relationship is established with the patient and his relatives from the first meeting. Empathic and supportive interventions
are essential in psychotherapeutic approach with cancer patient and his relative in out patient clinic. When working with cancer patients many
classical therapy techniques are restructured according to the illness, new therapies are developped by blending different therapy techniques.
Most frequently used in cancer patients can be counted as supportive psychothrerapy, cognitive behavioral therapy, brief psychodynamic
psychotherapy, supportive expressive psychotherapy, cognitive analytic psychotherapy, mindfulness therapy, meaning centered therapy,
relaxation and image based therapy, motivational interviews-counseling and dignity therapy. Cancer patients develop a variety of different
emotional, psychological, behavioral responses in prediagnostic examination phase, when they are told about diagnosis, acute treatment phase,
remission phase, recurrence-metastasis phase, terminal-palliative phase. Some of these responses are normal and intended to adaptation. In
this article psyhotherapeutic approach to the cancer patient and his relative will be explained with examples.
