Cinsel sorunların oldukça yaygın görüldüğü -her üç kişiden birinde- ve toplum sağlığını olumsuz etkilediği bilinmektedir. Geçmiş yıllara oranla günümüzde, cinsel sorun yaşayan kişiler daha kolay ve kısa sürede yardım arayışında bulunabilmektedirler. Poliklinikte hasta değerlendirip tedavi uygularken, zamanın kısalığı ve çok sayıda hasta görüyor olmak kısıtlayıcı bir durumdur. Ancak hangi hastalara, nasıl ve neleri kullanarak yardım edilebileceğini bilmek poliklinikteki uzmanın işini kolaylaştırabilir. Psikiyatristlerin biyolojik tedavilerdeki yetkinlikleri kadar psikoterapilerle ilgili de bilgi ve deneyimlerini artırmaları, cinsel sorunların çözümünde etkin olmalarını sağlayacaktır. Psikoterapötik yaklaşım, cinsel sorunla başvuran hastaları “psikoterapötik bakış” ile değerlendirmek, “psikoterapötik duruş” ile yönlendirmek ve mümkünse elde olan olanaklarla psikoterapi yöntemlerini kullanarak sağaltım düzenlemek şeklinde anlaşılabilir. Klinisyenlerin, hastaları poliklinikte psikoterapötik yaklaşımla ele almaları cinsel sorunların çözümünde önemli bir adımdır. Psikiyatristin donanımında, bu amaç için kullanabileceği üç araç bulunmalıdır: Cinsel anatomi-fizyoloji bilgisi, cinsel sorunların oluşumuna neden olan etkenlerin bilgisi ve tanı kılavuzu. Böylece sorunun gerçekten tedavi edilmesi gereken bir bozukluk olup olmadığını, olası nedenlerine ilişkin formülasyonu ve sağaltım için hangi se- çeneğin hem kendisi hem de hastası için en uygun yöntem olduğunu değerlendirebilecektir. Cinsel sorunla başvuran hastalara ya da çiftlere, uygun olanlar seçilerek psikoterapötik yaklaşım ile danışmanlık verilebilir; psikanalitik bakışın ve bilişsel-davranışçı tekniklerin kullanılacağı kısa süreli modifiye terapiler uygulanabilir. Çoğu cinsel sorunda, önyargıların aksine poliklinik ortamında uygulanabilecek zaman sınırlı psikoterapötik yaklaşım ile yüz güldürücü sonuçlar alınabilmektedir.

It is known that sexual problems are common -one in every three persons- and that they negatively affect community health care. Compared to previous years, individuals who experience sexual problems may seek help easier and earlier. While assessing and treating patients in the outpatient clinic, restricted time and the vast number of patients seen is a limiting situation. However, knowing how to choose patients and the kind of tools to be used may make the expert’s work easier. Psychiatrists increasing their knowledge and expertise about psychotherapies as well as their competence in biological treatment will allow them to be effective in finding solutions to sexual problems. Psychotherapeutic approach may be understood as assessing patients who consult with sexual problems with “psychotherapeutic view”, guiding them with “psychotherapeutic stance” and if possible, with the opportunities at hand, treating them by using psychotherapeutic methods. Approaching patients psychotherapeutically in the outpatient clinic is an important step in the solution of sexual problems. Within the scope of the psychiatrist’s equipment, there should be three tools which can be used for this purpose: knowledge of sexual anatomy-physiology, knowledge of factors which may cause formation of sexual problems and a guide for diagnosis. Thus he/she will be able to assess whether the problem is really a disorder which has to be treated, make a formulation about the possible causes of the problem and identify the option which offers the best method both for himself/herself and for the patient. Consultation through psychotherapeutic approach, and short-term modified therapies, during which psychoanalytic approach and cognitive-behavioral techniques are used, can be applied to the patients and couples who consult with sexual problems, and who are chosen as suitable. In most sexual problems, in contrast with biased appraisals, satisfactory results can be obtained through time limited psychotherapeutic approach that can be applied in the outpatient clinic.
