Bilişsel Davranış Terapisi (BDT) poliklinik koşullarında da kısa sürelerde kullanılabilecek başarılı bir yöntemdir. Ancak kısa süreli oturumların normal oturumlardan daha kolay olduğu gibi bir yanlış düşünceye kapılmamak gerekir. Süre daha kısıtlı olduğu için BDT uygulamaları hem daha hızlı hem de hedefe uygun bir şekilde yapılmalıdır. Kısa oturumlarda, terapiste düşen yük uzun oturumlardan daha fazladır. Terapideki ilk basamak terapistin açık bir iletişime sahip olmasıdır. Süre sınırlı olduğu için terapistler hastalarını dikkatli dinleyip, konuyu dağıtmadan, oturumların nasıl ilerleyeceğini, terapistin neler yapacağını ve hastadan beklenenlerin neler olacağını açıkça anlatmalıdır. Sonraki oturumda, sorunun tanımlanması ve sağaltım hedefinin belirlenmesi gerekir. Sorunun tanımı, hastanın bu yakınmalarının bir süredir olmasına karşın neden “şimdi” burada olduğunun anlaşılmasını sağlar. Hedef belirleme, terapilerdeki en önemli aşamalardan birisidir. İyi bir sağaltım hedefi, ana konu üzerinde odaklanılmasını ve zamanın uygun kullanılmasını sağlar. Koyulan hedefler, özgül ve ulaşılabilir olmalıdır. Belirsiz ya da yeterli planlanmamış hedefler, terapinin gücünü kaybetmesine neden olabilir. İyi planlanmış hedefler, hastaların konu dışına çıkmamasını, sağaltımın amacını hatırlamalarını ve ana konuya dönmelerini sağlar. Her oturumun başında hedefler konuşulup hedefe ne kadar yakın ya da uzak olduğu takip edilmelidir. Poliklinik koşullarında izlenen hastaların çok önemli bir bölümüne ilaç tedavisi de verilmektedir. Bu nedenle psikoterapi sırasında ilaç uyumu konusunun da ele alınması gerekmektedir. İlaç uyumu hastanın sağaltımının süreğenleşmeden kontrol altına alınmasında çok önemlidir. Bu yazıda kısa süreli BDT uygulamaları konusunda temel bilgiler paylaşılmaya çalışılmıştır.

The application of cognitive behavioral therapy at the psychiatry outpatient unit There is a common mistake that shorter cognitive behavioral therapy (CBT) sessions are easier than the standard duration CBT sessions. However, because of time restriction in the short CBT sessions, the therapist has to be more sharp and careful while using his or her technique. Short CBT sessions require more workload than the standard CBT sessions. The first step in the short CBT session is to have an open communication between the therapist and the patient. Since the time is limited the therapist has to listen carefully to what the patient said and describe how therapy sessions will be held and ask what are the things that therapist going to demand from patient. In the next session, defining the problem and setting the goal of the therapy is crucial. Setting goals is the most important stage in the therapy sessions. A good set of goals allows the therapist to focus on the main problem, and use his or her time efficiently. The therapist and the patient have to set goals that are unique and achievable. Unclear or inadequate goals lead to a weakening of the therapy. On the other hand a good set of goals makes patients not to lose focus on the main problem and never forget the main aims of the therapy. In every session, the distance between the goal and, the current situation in the therapy has to be checked. The majority of the outpatients are using medications, and because of that, drug compliance is an important thing to keep an eye on in therapy sessions. To conclude, CBT can be used in outpatient units. In this article we try to explain the main aspects of short term CBT sessions.
