Engel’in biyopsikososyal (BPS) model tanımı, sağlıkla ilgili her kararda ve girişimde, her üç türden etmenin, her zaman, hemen hemen eşit derecede etkili olduğu ilkesine dayalıdır. Psikoterapötik yaklaşım, başlı başına bir sağaltım yöntemi olan psikoterapiden farklı olarak, hasta – hekim ilişkisinde tanı değerlendirmesinden başlayarak mevcut olması gereken bir bilgi, beceri ve tutum kümesine işaret eder. Bu özelliğiyle, bütüncül BPS modele uygun bir tanı ve sağaltım sürecinin vazgeçilmez bir bileşenidir. Poliklinikte psikiyatri uygulamasının kendine özgü olanakları ve güçlükleri vardır. Başlıca güçlüklerden biri olan zaman kısıtlılığı, sağaltımın tanımlı psikoterapi yöntemlerinden birine dayalı olmasına çoğu zaman engeldir. Psikoterapötik yaklaşım, tanı değerlendirmesinde etkin ve eşduyumlu dinlemeyi, görüşme tekniklerine hakimiyeti, izlem ve sağaltım sürecinde düzenleme gerektiğinde bunu fark edebilecek düzeyde klinik sezgiye sahip olmayı kapsar. Bunlar sadece kuramsal bilgiyi değil, aynı zamanda bilgiyi iyi kullanabilecek deneyime sahip olmayı gerektiren beceri ve tutumlardır. Bu makalede, poliklinik koşullarında BPS anlayışa uygun psikoterapötik bir yaklaşımı sürdürebilmek için gerekli olan klinik bilgi, beceri ve tutumlar özetlenmekte, sık rastlanan hatalar ve pratik öneriler olgu örnekleriyle aktarılmaktadır.

The biopsychosocial (BPS) model, as defined by Engel, is based on the principle that every health-related task involves biological, psychological, and social dimensions in every single case, and their contribution to the clinical presentation is always almost equivalent. A therapeutic element in the physician – patient relationship is therefore indispensable in the holistic BPS model. Unlike psychotherapy, which is relevant only to treatment, a psychotherapeutic approach includes a group of attributes which includes clinical skills, experience, and attitudes that operate beginning with the initial diagnostic assessment. Outpatient psychiatry in the general hospital setting has its convenient aspects, compared to other contexts such as private practice or community mental health centers. However, the challenges usually outnumber the opportunities, not the least of which is the time constraint. This is one of the conditions where the physician’s psychotherapeutic skills and the biopsychosocial approach prove most helpful. This article will outline the clinical knowledge, skills and attitudes that are needed to establish and maintain a proper management of outpatient cases. Recommendations and common mistakes will be presented with the help of case vignettes.
