Epileptik Olmayan Çocuklarda Tipik Epileptiform EEG Anomalileri: Klinik ve Elektroensefalografik Değerlendirme

Kliniğimiz EEG laboratuvannda 1993-94 yıllan içinde epilepsi ve epilepsi kuşkusu dışında nedenlerle, 16 yaş altında toplam 2194 olguda EEG incelemeleri yapılmış; bu olgulardan 72 tanesinde (3.3 %) tipik epileptiform anomali saptanmıştır. Yaşlan 7.2±2.6 (min: 1.5; max: 15) olan olguların 46'sı erkek, 26'sı kızdı. Olgular EEG anomalisinin özelliğine göre gruplandınldığmda; 10 olguda jeneralize tipte anomali saptandı, bu olgulann biri hariç (davranış bozukluğu), tümü nörolojik hastalık nedeniyle incelenmişti. EEG anomalisi multifokal olarak ele alman 19 hastanın tümü çeşitli nörolojik hastalıklarlar nedeniyle gönderilirken, unifokal anomali gösteren 43 hastanın 16 tanesi farklı nöropsikiyatrik nedenlerle (6 kekemelik, 4 enürezis noktürna, 3 tik bozukluğu, 2 otizm, 1 davranış bozukluğu) EEG laboratuvanna başvurmuştu. Temel aktivitenin yetersiz bulunması jeneralize grupta ve multifokal grupta yaklaşık 1/3 oranındayken, unifokal anomali grubunda diğer iki gruba oranla anlamlı şekilde düşüktü (p< 0.03). Fokal anomali gösteren olgulann lokalizasyonlan değerlendirildiğinde 87 % olguda santral ve temporal bölgeler ile ilişkili odak bulunduğu dikkati çekti.
Klinikleri ortalama 8 ay süre ile izlenen 37 olgunun hiçbirisinde bu sınırlı süre içinde epilepsi gelişmedi. flEG'leri takip edilen 32 olgudan (19: 6 aydan kısa; 13: 6 aydan uzun) 8 tanesinde EEG'nin normale döndüğü saptanırken, geri kalan olgularda amplitüd, morfoloji, sıklık ve lokalizasyon açılarından değişkenlikler görülmekle birlikte epileptiform anomali devam etti.
Typical Epileptiform Eeg Discharges In Nonepileptic Child¬ren: Clinical And Electroencephalographic Evaluation
In our EEG laboratory between the [year]s 1993-94, a total of 2194 cases under the age 16 had been underwent EEG investigations, for causes other than epilepsy and suspected epilepsy; 72 (3.3 %) of them had typical epileptiform EEG discharges. There were 46 males and 26 females in these group and mean age was 7.2±2.6. All but one (conduct disorder) of the 10 cases with generalised epileptiform EEG discharges, were sent to the EEG laboratory for neurological diseases. Also all of the 19 cases with multifocal EEG abnormalities had been investigated for various neurological disorders, whereas 16 of the 43 cases with unifocal EEG discharges were refened for different neuropsychiatric disorders (6 cases with stuttering, 4 enuresis nocturna, 3 with tics, 2 autistic patients, 1 with conduct disorder). Background EEG activity was found to be abnormal in 1/3 of the patients from generalised and multifocal abnormality groups, whereas in the group with unifocal EEG discharges background activity was spared when compared with the 2 other groups (p< 0.03).
Analysis of the location of the focal abnormalities disclosed foci related to the central and temporal regions in 87 % of the cases. None of the 37 cases, who had been followed for a mean interval of 8 [month]s, developed epilepsy in this limited period. Of the 32 cases who were available for repeated EEG examinations, 8 had normalisation of the EEG, whereas in the rest of the cases the abnormality persisted with impressive variation in amplitudes, morphology, location and frequency.
