Patogenezi henüz aydınlatılamaımş olan Alzheimer hastalığının (AD) tipik bulgularından biri senil plaklarda amiloid birikimidir. Akut faz proteinlerinin Amiloid protein prekürsör'ün degradasyonuna yol açarak beta / A4 fragmanının ortaya çıkmasına, dolayısıyla amiloid birikimine yol açtığı ileri sürülmektedir. Bu kuram doğrulanırsa klinik uygulamaya girebilecek immunosupresan tedaviler AD tedavisinde yeni bir dönem başlatabilir. Günümüze dek yapılmış çalışmalarda AD tanısı almış kişilerde IL-1 ve IL-6 serum değerleri kontrol gruplarından farklı bulunmamıştır. AD hastalarında BOS İL-1 beta değerleri, bu kuramı destekler yönde yüksek bulunmuştur. Ancak bu çalışmalar kontrol gruplarına merkezi sinir sisteminde AD benzeri dejeneratif değişiklikler olabilecek hastaların dahil edilmesi ve periferik bir patolojiyi dışlamak amacıyla beyin omurilik sıvısı (BOS)/serum interlökin indekslerinin tayin edilmemesi açısından eleştirilebilir. Çalışmada "akut faz yanıtı" hipotezini sınamak için, Alzheimer tanısı konmuş 8 hastanın BOS ve serum IL-1 ve IL-6 düzeyleri, organik beyin sendromu saptanmayan 12 hastadan oluşan kontrol grubu ile karşılaştınlmıştır. Her iki • grupta da BOS, serum ve indeks değerleri birbirine yakın bulunmuştur. BOS interlökin düzeyleri ve demans şiddeti arasında ise pozitif bir korelasyon saptanmıştır. Bu bulgu "akut faz yanıtı" kuramını destekler gözükmektedir.

Alzheimer's Disease (AD) is a disorder of unknown etiology with typical manifestations such as senile plaques. It has been hypothesized that the amyloid in senile plaques is formed and deposited due to degradation of Amyloid Protein Precursor (APP) by acute phase proteins which led to beta / A4 formation. If this hypothesis is proven to be true, a new era in treatment of AD may begin by clinical use of imrnunosupressant drugs. Studies that have been conducted up to date were insufficient to show any difference between serum IL-1 and IL-6 levels of patients with AD and controls. IL-1 beta levels were found to be elevated in AD patients, which seem to be consistent with this hypothesis. These studies can be critized because of the control groups which included the patients who might have similar degenerative lesions as AD. Besides, these studies has not taken cerebrospinal fluid (CSF) / serum interleukin index into consideration to exclude peripheric patology. To investigate "acute phase response" hypothesis, 8 patients with AD and 12 controls without organic mental syndrome were compared in terms of IL-1 and IL-6. Serum, cerebrospinal fluid and index levels of IL-1 and IL-6 were found to be similar in both groups. However, there was positive correlation between the severity of dementia and CSF interleukin levels which seemed to support "acute phase response" hypothesis.
