Bu yazıda, inmede atriyal fibrilasyonun önemine, epidemiyolojisine, mekanizmasına ve sağaltıma değinen son çalışmalar gözden geçirilmiştir. Son zamanlarda tamamlanan 6 çalışma, inmede antikoagulasyon ve aspirinin primer ve sekonder korunmadaki önemi üzerinedir. Bu çalışmalarda standardizasyon açısından, protrombinin enternasyonel normalize edilmiş oranı (INR) kullanılmış olup, bu oran değişik tromboplastin duyarlılık düzeylerini düzeltmektedir. Primer korunma çalışmalarında aspirinin ve antikoagulanlarm etkili olduğu gösterilmiştir. Sekonder korunma çalışmalarında, antikoagulanlarm aspirinden daha etkili olduğu gösterilmiştir. Bugün için, antikoagulasyonun optimal miktarı ve kullanma süresi tam açık değildir. Gelecekte, SPAF-III çalışması, Danimarka AFASAK-II çalışması ve primer korunma üzerine yapılan PATAF çalışması, atriyal fibrilasyonda optimal terapötik yaklaşımların ortaya konmasında yardımcı olacaktır.

In this review, we evaluated recent studies which emphasized the epidemiology, mechanism, and treatment of atrial fibrillation. The recent six large clinical trials have investigated the value of anticoagulation and aspirin in the primary and secondary prevention of stroke. In these studies, they used prothrombin time international normalized ratio (INR), which corrects for variable thromboplastin sensivities. The primary prevention studies showed that both anticoagulants and aspirin are effective. For secondary prevention, anticoagulants were significantly more effective than aspirin. For present, it is not clear the optimal intensity and duration of anticoagulation. In the future, the SPAF-III trial, the Danish AFASAK-II trial, and ongoing primary prevention study, the PATAF trial, may help to establish the optimal therapeutic approach in the prevention of thromboembolic complications in atrial fibrillation patients.
