Nöropsikolojik olarak global dikkat sisteminin bozukluğu ile açıklanabilecek akut konfüzyonel durumda düşünce içeriğindeki bozukluk, sistematize bir hezeyan şeklini de alabilir. Bu yazıda akut konfüzyonel durum tablosuyla kliniğimize başvuran, günler içinde "bütün yakınlarının üpkılan olan sahteleri ile yer değiştirdiği" temalı Capgras sendromu özellikleri göstermeye başlayan bir olgu bildirilecektir. Premorbid nörolojik ya da psikiyatrik öyküsü bulunmayan, bu açıdan soygeçmişi de özellik göstermeyen 53 yaşmda, sağ elli kadın hasta bir haftadır süregelen tedirginlik, uykusuzluk, çevreyle iletişimsizlik yakınmalanyla başvurdu. Nörolojik muayenesinde dikkati her türlü dış uyaranla çelinebilen, motive edilemeyen hastanın kayıt belleğinin ve orientasyonunun ileri derecede bozuk olduğu görüldü. Kranyal MRI, beyin Tc99 HMPAO SPECT'i, EEG, BOS, biyokimyasal, endokrinolojik, hematolojik incelemeleri normal olan hastada günler içinde Capgras hezeyanı özellikleri hakim bulgu haline geldi. Buna zamanla depresif, mhilistik bir temayı andıran "cezalandırılma" düşünceleri eşlik eder oldu. Psikotik depresyon olarak kabul edilen hasta antedepresan tedavi ile 2 ay içinde tam düzelme gösterdi. Bu olgu dolayısıyla ortak özellikleri frontal sistem bozukluğu olan nörolojik hasarlarla, "fonksiyonel" psikiyatrik hastalıklarda eş sıklıkta bildirilmiş olan Capgras sendromu gözden geçirilmiş ve nöroloji ve psikiyatri arasındaki köprü denebilecek olan frontal sistem işlevleri açısından yorunlanmıştır.

In acute confusional state which is neuropsychologically a primary disorder of global attention system, the accompanying disorder in thought content can take the form of a systematic delusion. In this paper a case, presenting with acute confusional state and developing the properties of Capgras syndrome within days, claiming "all her relatives have been substituted by identical imposters" is presented.
A 53 [year]s old, right handed woman, without any premorbid neurological or psychiatric personal and family histories, was admitted with restlesness, sleep deprivation and apathy starting acutely a week before. In mental state examination she was easily distracted, showed very poor motivation; short term memory, temporal and spatial orientation were all severely impaired. Neurological examination was otherwise normal. Cranial MRI, Brain Tc99 HMPAO SPECT, EEG, spinal tap, biochemical, endocrinological and hematological investigations were within normal limits. Within days features of Capgras delusion became the predominant sign. In time thoughts of punishment resembling a depressive nihilistic delusion developed. A diagnosis of psychotic depression was made and the patient recovered fully in two [month]s with antidepressant medication.
A review of the relevant literature shows that the Capgras syndrome has been reported somewhat equally in structural brain lesions and "functional" pschoses. It is suggested that the frontal system may be the common denominator.
