Kronik Karaciğer Hastalıklarında Multimodal Uyarılmış Potansiyeller

Hepatik ensefalopatide değişik oranlarda multimodal uyarılmış potansiyellerin (MMEP) subklinik hepatik ensefalopati (SHE) tanısı koymada da kullanılabileceği ileri sürülmektedir. Çalışmamızda klinik ensefalopatisi olmayan 11 postnekrotik siroz ile 12 alkolik siroz olgusu yanı sıra ensefalopati tablosundaki 9 postnekrotik siroz ve 7 alkolik siroz olgusunda MMEP değerlendirilmiş, elde edilen sonuçlar 20 normal kontrol ve karaciğer patolojisi olmayan 10 kronik alkolik olgunun sonuçlan ile karşılaştınlmıştır. Görsel uyarılmış potansiyeller (VEP) ile sadece ensefalopati tablosundaki olgularda patoloji saptanırken, işitsel uyarılmış potansiyeller (AEP) ensefalopatili ve ensefalopatisiz tüm olgularda normaller ile karşılaşıldığında istatistiksel olarak anlamlı anlamlı şekilde bozuk bulunmuştur. Somatosensoriel uyarılmış potansiyel (SEP) patolojisi egemen olarak ensefalopatili olgularda gözlenmiştir. Sonuç olarak, hepatik ensefalopatide tüm uyarılmış potansiyellerde bozukluk söz konusu iken, SHE'yi ortaya koymada en duyarlı yöntemin AEP olduğu kanısına varılmıştır.
Multimodal Evoked Potentials In Patients With Chronic Liver Disease
Varying degrees of multimodal evoked potential (MMEP) abnormalities have been reported in patients with hepatic encephalopathy. It has J been suggested that MMEP's can be used in the assesment of subclinical hepatic encephalopathy as well.
In the study MMEP recordings were examined in 11 postnecrotic cirrhotic and 12 alcoholic . cirrhotis patients without encephalopathy in 1 addition to 9 postnecrotic cirrhotic and 7 alcoholis cirrhotic patients with encephalopathy. The results were compated with the results gathered from 20 normal controls and 10 chronic alcoholic patients without any liver pathology. VEP abnormalities were detected only in patients with encephalopathy. AEP's on the other hand were disturbed in all patients with or without encephalopathy when compared with the normal controls, SEP abnormalities were prominent only in patients with encephalopathy.
In conclusion, AEP was found to be most valuable investigation for the objective assesment of SHE in patients with cirrhosis.
