Kronik Kokain Kullanan Bir Hastada Subaraknoid Kanamanın Kötü Seyri: Bir Olgu Sunumu

Kokain kullanımında iskemik inme, intraparenkimal kanama ve subaraknoid kanama (SAK), epileptik nöbet gibi çeşitli nörolojik komplikasyonlar görülebilir. Kokain sebebiyle gelişen hipertansiyon; serebral dolanımda hemodinamik stress oluşturarak hem anevrizmanın erken gelişimine hem de kanamanın daha genç yaşlarda görülmesine neden olur. Ayrıca gelişen arteriopati ile de SAK'ta olumsuz prognoza yol açar. Çalışmada SAK tanısı ile incelenen ve kronik kokain kullanım öyküsü olan bir olgu sunulmaktadır. Anjiogramda multipl anevrizma ve arteryel düzensizlikler saptandı. Vazospazm nedeni Üe kanamanın 19. gününde ameliyat edildi. Postoperatif 6. güngeçirilen gastrointestinal sistem (GİS) kanamasına bağlı hipotansiyon nedeni ile iskemik bulguları ilerledi ve 12. gün GİS kanaması tekrarladı. Onbeşinci gün miyokard infarktüsü (Mİ) gelişti ve bu komplikasyonlarm ardından 40. gün intraventriküler kanama geçirerek Reanimasyon kliniğine yaünlan olgu 51. gün kaybedildi. Kokain kullanımının oluşturduğu sistemik damar hasan ve hipertansiyon, SAK'lı olgularda farklı bir prognoza sahip olabilir. Bu nedenle, kokain kullanıcılarda SAK'ın doğal seyrinin daha farklı, mortalite oranlarının beklenenin üzerinde olduğu unutulmamalıdır.
Poor Prognosis of Subarachnoid Hemorrhage Associated With Chronic Cocaine Usage: A Case Report
Neurologic complications including ischemic stroke, seizure, intraparenchymal and subarachnoid hemorrhage (SAH) may be seen due to cocaine abuse. Cocaine-induced hypertension causes hemodynamic stress in the cerebral circulation and thereby precipitates early formation and rupture of the aneurysm. Cocaine abuse appears to be a significant negative prognostic factor in the natural history of SAH.
We report a patient with SAH associated with chronic cocaine abuse Angiography showed multiple aneurysms and arterial irregularities. The patient was operated on the 19th day of SAH due to prolonged vasospasm. Ischemic deficits progressed due to the hypotension on the 6th postoperative day because of the gastrointestinal bleeding. Gastrointestinal bleeding recurred on the 12th day. Myocardial infarction developed on the 15th postoperative day and intraventricular hemorrhage on the 40th day. The patient died on the 51st day in the intensive care unit.
Systemic vascular damage and hypertension may cause unfavorable prognosis in cases of SAH due to cocaine abuse. For these reasons, it should be kept in mind that the natural prognosis of SAH is different and the mortality rates are higher in the cocaine addicted patients.
