Aydınlatılmış Onam Uygulamasında Psikiyatrik Konsültasyonun Yeri

Çağdaş hekim-hasta ilişkisinde babaca (paternalist) tutumun giderek yerini paylaşımcı tutuma bırakması, söz konusu ilişkinin Hippocrates'ten bu yana dayanmakta olduğu güven öğesini tartışmalı duruma getirmiştir. Günümüzde hekimin tümgüçlülüğüne ve bu gücü her zaman hastasının yararına kullanacağı yolundaki genel kanıya dayanan Hipokratik güven temelinin yerini, özerkliğe saygı ilkesine dayanan güven almaktadır. Temel amaçlanndan biri, hastalık nedeniyle sınırlan daralmış bulunan özerkliği onarmak olan tıpta özerkliğe saygı ilkesi, aydınlatılmış onam öğretisi ile uygulama alanına yansımaktadır. Aydınlatılmış onam; hastanın hastalığını ve bu nedenle kendisine uygulanması düşünülen tanı ve tedavi yöntemlerini, bu yöntemlerin yararlarını, olası zararlarını, onlara seçenek oluşturan öteki yöntemleri, seçenek yöntemlerin olumlu ve olumsuz yönlerini bilerek kendisiyle ilgili kararlar vermesi anlamına gelmektedir. Öğretiye göre söz konusu bilgilendirmeyi hekimin yapması ve bu bilgilere dayanarak hastanın verdiği karara saygı göstermesi gerekmektedir. Hekimin böyle bir uygulamayı yapabilmesi için hekimlik bilgilerine ek olarak başka bazı bilgi ve becerilerle donanmış olması gerekmektedir. Aydınlatılmış onamın alınmasında temel öneme sahip becerilerle ilgili eğitimin tıp eğitiminin bir parçası durumuna gelmediği ülkemizde, uygulamada sık sık sorunlarla karşılaşılmaktadır.
Bu sorunların başında hekimin aydınlatma ve onam alma işini tümüyle, iletişim ve empati becerileri konusunda belirli bir eğitim almakta olan ruh hekimlerine bırakması gelmektedir. Hekim, özellikle kararlarını hastasına kabul ettirmekte güçlük çektiğinde, psikiyatri konsültasyonu isteyerek sorunu çözmeye çalışmaktadır. Bu makalede konsültasyon-liyezon psikiyatristinin çağdaş hekimlik uygulamasını karşılıklı katılıma dayak hekim-hasta ilişkisi biçiminde yapılandırmada üstlenebileceği işlevler tartışılacaktır.
The Role of Psychiatric Consultation in Receiving Informed Consent
As the paternalistic attitudes are replaced by the mutual attitudes in the contemporary physician-patients relationship, trust which has been one of the most important and basic components of it since Hippocrates, has begun to be questioned. Nowadays the place of Hippocratic trust, which stems from the physicians' omnipotence and the general opinion which accepts that (s)he will always use this power for the beneficience of the patient, is taken over by the trust which is based on the principle of respect for autonomy. The principle of respect for autonomy is expressed by the doctrine of informed consent in the routine practice of medicine. Medicine is a discipline whose main purpose is restoring autonomy that turns out to be limited as a result of illness. Informed consent means that the patient makes decisions about himself after being informed about his diagnosis, the diagnostic and therapeutic procedures he will probably need to undergo, alternative procedures and the risks and benefits of all the procedures in question. According to the doctrine, the necessary information should be given by the physician and he should also respect the decision of the patient which is based on this information. The physician should be equipped by some kind of specific knowledge and skills besides the core knowledge of medicine, in order to carry out this task. As the education of basic skills which are necessary for receiving informed consent, is not included in medical education in our country, some problems arise frequently in the daily practice.
The main issue is the physicians' tendency of leaving the task of informing patients and receiving their consents to psychiatrists, whose education consists of the skills for communication and empathy to a certain extent. Physicians try to solve then-problems by asking for a psychiatric consultation, especially when they have difficulty in receiving patients' acceptance for their decisions. In this article, the potential functions of consultation-liaison psychiatry in reconstructing contemporary physician-patient relationship as a mutual relationship will be discussed.
