Psikotik bozukluklar ve duygudurum bozuklukları ani başlangıçlı olabileceği gibi, sıklıkla sosyal/ emosyonel içe çekilme, duygudurumda değişiklik, anksiyete, eşik altı tuhaf algısal ve bilişsel içerik gibi öncü belirtilerle yavaş biçimde kendini gösterebilmektedir. Bahsedilen öncü belirtilerin olduğu prepsikotik süreç çoğunlukla bilişsel ve duygulanımsal olarak kendine özgü özelliklere sahip bir dönem olan ergenlik dönemine rastlamaktadır. Bu nedenle tablo karışık görünür ve tanı koymak zorlaşabilir. Bu olgu sunumunda, DSM-5 sınıflandırmasına göre Bipolar Bozukluk, Depresif atak ve Dikkat eksikliği Hiperaktivite Bozukluğu ön tanıları ile servise yatırılan ve psikodinamik olarak da kimlik krizinde olduğu düşünülen bir ergen hasta tartışılmıştır. Hastanın öncü belirtileri dikkatlice ele alınmış ve komorbid hastalıklarının da tanısı ve tedavisi ile işlevselliği belirgin düzeyde artmıştır. Eş zamanlı yapılan psikososyal müdahaleler, grup ve bireysel psikoterapiler, uğraş terapileri ile ergenin kimlik gelişimi sürecinde kendisine destek olunmuştur. Ayrıntılı inceleme ve biyopsikososyal bütüncül yaklaşım doğrultusunda düşünülecek ön tanılar ve yapılacak müdahalelerin genç hastaların bütün bir hayatını değiştireceği, işlevselliğini arttıracağı unutulmamalıdır

Psychotic and affective disorders may appear either suddenly or slowly with some prodromal symptoms such as social/ emotional withdrawal, affective changes, subtreshold level of odd perception and/or cognition. The prepsychotic period with prodromal symptoms often overlap with adolesence which has its own special cognitive and affective changes. Therefore, clinical presentation and diagnostic process become more complicated. In this case report, an adolescent patient who was admitted to the service with the preliminary diagnoses of Bipolar Disorder, Depressive Attack and Attention Deficit Hyperactivity Disorder according to DSM-5 classification and psychodynamically thought to be in an identity crisis was discussed.The prodromal symptoms of the patient were carefully assessed and his functionality increased significantly by the diagnosis and treatment of comorbid disorders. At the same time, psychosocial interventions, group and individual psychotherapies and occupational therapies helped the identity development of the adolescent. It should be remembered that prediagnostic process and biopsychosocial assessment with detailed investigation may highly change the whole life of patients.
