Gerekçe: Sıklıkla bedensel hastalık belirtileri ile karı­şabilen depresif belirtileri ilk basamakta farkedebile-cek kişi, bedensel hastalığın tedavisini sürdüren he­kimdir. Bu çalışmada, bedensel hastalığı olan ve psi­kiyatri konsültasyonu sonucunda herhangi bir depre­sif bozukluk tanısı konmuş hastalarda hangi tür dep­resif belirtilerin konsültasyonu isteyen hekimin daha fazla dikkatini çektiğinin ve konsültasyon istek nede­nini oluşturduğunun incelenmesi amaçlanmıştır. Yöntem: 1996 yılı boyunca İstanbul Tıp Fakültesi Hastanesi yataklı servisleri tarafından Psikiyatri Anabilim Dalı Konsültasyon-Liyezon Bilim Dah'ndan istenilen 840 konsültasyon taranmış ve herhangi bir depresif bozukluk tanısı konmuş olan 303'ü değer­lendirmeye alınmıştır. İstek nedenleri duygusal, bi­lişsel, güdüsel, davranışsal ve bedensel alt başlıkları altında toplandıktan sonra dağılımları incelenmiştir. Sonuçlar: Hastaların %54'ünde depresif uyum bo­zukluğu, %14'ünde major depresif bozukluk, %25'in-de başka türlü adlandınlanıayan depresif bozukluk ve %7'sinde distimik bozukluk saptanmıştır. Duygu­sal belirtiler %52, bedensel belirtiler %32, davranış­sal belirtiler %12, güdüsel belirtiler %10, bilişsel be­lirtiler ise %7 oranında saptanmıştır. Major depres­yon ve depresif uyum bozukluğu tanısı konmuş olan hastalarda güdüsel belirtilerin daha fazla görüldüğü saptanmıştır. Duygusal belirtilerin de yaşlı hastalar­da daha fazla görüldüğü bulunmuştur. Tartışma: Psikiyatri dışı hekimlerin kolay farkedile-bilen duygusal belirtileri tanıdıkları, ancak sadece hasta ile daha fazla iletişime girildiğinde aynşürıla-bilecek bilişsel belirtileri tanıyamadıkları dikkati çekmektedir.

Objective : The medical doctor, following the treatment of the medical illness is the one who can realize the depressive symptoms of the patient at the first step. To assess which kind of depressive symptoms are noticed and which take part in the consultation request by the medical doctor, the following study was designed. Method: 840 psychiatric consultations which were requested from Psychiatry Department Consultation - Liaison Unit by the impatient clinics in Istanbul Me­dical Faculty were scanned. 303 of them which men­tioned of diagnosis of any kind of depressive disorder were included in the study. Reasons of requests we­re grouped as emotional, cognitive, motivational, be­havioral, and somatic subgroups and their variations in different depression states were evaluated. Results: Depressive adjustment disorder was diag­nosed in 54%, major depressive disorder in 14%, not otherwise specified depressive disorder in 25%, and dystiiymic disorder in 7% of the patients. Emotional features were found in 52%, somatic features in 32%, behavioral features in 12%, motivational featu­res in %10 and cognitive features in 7% of the con­sultation request reasons.
Discussion: It is observed that medical doctors can realize emotional symptoms that can be noticed eas­ily (crying etc.), but they cannot recognize especially cognitive symptoms which require more com­munication with the patient.
