Parkinson hastalığında işitsel olaya bağlı potansiyeller

Bu çalışmada, idiopatik Parkinson hastalığı (PH) tanısı ile izlenen 30 hasta (21E, 9K) ve 10 (4E, 6K) normal gönüllü denekden oluşan kontrol grubu, önce kısa mental durum testi (KMDT) uygulanıp sonra işitsel olaya bağlı potansiyellerin P300 komponenti çalışılarak, PH'da kognitif fonksiyonların değerlendirilmesi amacıyla karşüaştınlmıştır. Hoehn - Yahr sınıflamasına göre PH grubundaki hastaların 20'si Evre I, 10'u Evre ITde idi. PH grubunun KMDT ortalaması kontrol grubundan düşük olmasına rağmen aralarındaki fark anlamlı bulunmadı. PH grubundaki hastaların Fz ve Cz'den kaydedilen Nl, P2, N2 ve P3 latanslar normallerden daha uzun bulunmasına rağmen aralarındaki fark istatistiksel olarak anlamlı değildi. PH grubunda Cz'den kaydedilen N2/P3 amp-litüdü normallere göre sınırda düşük bulundu (p: 0.0528). Fz'den kaydedilen N1/P2, P2/N2, N2/P3 amplitüdleri ve Cz'den kaydedilen N1/P2, P2/N2 amplitüdleri de normallerden düşük olmasına rağmen aralarındaki fark istatistiksel olarak anlamlı bulunmadı. PH grubunda Cz'den kaydedilen N2/P3 amplitüdü ile hastaların yaşı arasında negatif korelasyon saptandı. Çalışmamız, PH'da işitsel olaya bağlı potansiyelin P300 komponentinde etkilenme olabileceğini ancak bu etkilenmenin tedavi başlanmamış, "off döneminde ve ileri evrelerdeki hastalarda daha belirgin olabileceğini düşündürmüştür.
Auditory Event - Related Potentials In Parkinson's Disease
In this study, 30 patients (21 male, 9 female) with idiopathic Parkinson's disease (PD) and 10 healthy volunteers (4 male, 6 female) were compared in order to assess the cognitive functions in PD by giving the Short Test of Mental Status (STMS) and evaluating the P3 component of auditory event-related potentials. In the PD Group, 20 patients were in stage I and 10 in stage II according to the Hoehn-Yahr scale. Although the mean of the STMS score in PD group was lower than that of normal controls, the difference was not statistically significant. The Nl, P2, N2 and P3 latencies recorded from Fz and C2 were longer in PD group, but the difference was also not statistically significant. Comparing with the controls, lower N2/P3 amplitudes recorded from Cz in PD group showed a tendency toward significance (p: 0.0528). The N1/P2, P2/N2, N2/P3 amplitudes recorded from Fz and the N1/P2, P2/N2 amplitudes recorded from Cz in PD group were found to be lower than that of the control group, yet the difference was unsignificant. There was a negative correlation between the N2/P3 amplitude recorded from Cz and the age of PD patients. These results suggest that the P300 component of auditory event-related potentials may be effected in Parkinson's disease; but further studies with untreated PD patients being in "off periods and advanced stages may reveal more reliable information.
