Dikkat türlerinin şizofreni alt gruplarına göre değişimi

Amaç: Çalışmanın amacı, şizofreni tanısı almış olan olgularda dikkatin, Türk kültürüne standardizasyonu yapılmış ölçme araçları ile çok-yönlü incelenmesidir. Bir diğer amaç, şizofreni alt gruplarının dikkatin türleri açısından farklılaşıp farklılaşmadığını belirlemektir.
Yöntem: Çalışmada kullanılan nöropsikolojik testler; Stroop Testi TBAG Formu, İşaretleme Testi (İT), Wechsler Bellek Ölçeği Geliştirilmiş Formu (WMS-R) ve Görsel İşitsel Sayı Dizileri Testi B Formu (GİSD-B) olmuştur. Çalışmanın deney grubunda, 20-54 yaş aralığında, iki eğitim düzeyinden gelen (9-11 yıl ve 12 yıl ve üstü), 32 şizofreni hastası (16 pozitif, 16 negatif alt grup) bulunmaktadır. Kontrol grubunu ise aynı yaş aralığında ve eğitim düzeyinde, herhangi bir
psikopatoloji grubuna girmeyen 32 normal denek (16 kadın, 16 erkek) oluşturmuştur.
Bulgular: Çok Değişkenli Varyans Analizi (MANOVA) sonuçlarına göre; şizofreni grubu ile normal grup arasında söz konusu testlerin dikkate ilişkin puanları açısından anlamlı fark olduğu bulunmuştur. Lojistik Regresyon Analizi sonuçları; Stroop Testi TBAG Formu ile İT testlerinin dikkate ilişkin puanlarının iki alt grubu ayırdedebildiğini göstermiştir. WMS-R ve GİSD-B'nin dikkate ilişkin puanlarının ise bu iki alt grubu ayırdetmede yeterli olmadığı
bulunmuştur.
Sonuç: Çalışma, odaklanmış dikkat (Stroop Testi TBAG Formu) ve sürekli dikkatin (İşaretleme Testi) şizofreninin alt gruplarını yordamada kullanılabileceğine, dikkat uzamının ise böyle bir ayırıma katkısının zayıf olduğuna işaret etmiştir. Bu bulgular, Strooop Testi ve İşaretleme Testi puanlarının, şizofreninin alt grup sınıflamasına katkıda bulunacağını ortaya koymuştur.
The study of attention according to subgroups of schizophrenia
Objective: The goal of the study was to investigate attentional disturbances using appropriately chosen neuropsychological tests in schizophrenia patients and to determine whether subgroups of schizophrenia can be discriminated on the basis of test scores.
Method: The neuropsychological tests used in the study were Stroop Test TBAG Version, Cancellation Test (CT), Wechsler Memory Scale-Revised (WMS-R) and Visual Aural Digit Span Test (VADS). The study was comprised of two groups. The group with schizophrenia consisted of 32 patients (16 from positive and 16 from negative subgroup) from the age range of 20-54 [year]s and from two levels of education (9-11 [year]s and 12 [year]s or more). In the control group
there were 32 reportedly healthy subjects.
Results: Multivariate Analysis of Variance (MANOVA) showed a significant difference between the two groups on the basis of Stroop Test TBAG Version, CT, WMS-R and VADS scores. Logistic Regression Analysis showed that subgroups of schizophrenia can be predicted from Stroop Test TBAG Version and CT scores. All WMS-R scores distinguished the subgroups but WMS-R scores specifically related to attention did not do so.
Conclusion: In the study, the scores of the subjects in the negative subgroup was found to be significantly lower than those in the positive subgroup. The subgroups could be distinguished on the basis of Stroop Test TBAG Version and CT. These findings showed that Stroop Test TBAG Version and CT scores can contribute to subgroup classification of schizophrenia.
