Obsesif kompulsif bozuklukta tedaviye cevapla ilişkili etkenler

Amaç: Bu çalışmada obsesif kompulsif bozuklukta (OKB) farmakolojik tedaviye yanıta etki eden faktörlerin araştırılması
amaçlanmıştır. Yöntem: Çalışmaya DSM-IV’e göre OKB tanısı konmuş olan 60 hasta (39 kadın ve 21 erkek) alınmıştır. Tüm
hastalara, Yale-Brown Obsesyon Kompulsiyon Ölçeği (Y-BOCS) uygulanmıştır. Klinik değerlendirme sonrasında hastalara
ilaç tedavisi başlandı (28 hastaya fluvoksamin 100-300 mg/gün; 15 hastaya fluoksetin 20-80 mg/gün ve 17 hastaya
sertralin 100-200 mg/gün). Tedavi dozları hastanın klinik durumuna ve yan etkilere göre ayarlandı. Tedaviye yanıt üçüncü
ayın sonunda Klinik Global İzlenim Ölçeği (KGİ) ile değerlendirildi. Çok daha iyi ve daha iyi olanlar cevap verenler olarak
tanımlandı. Tedavi yanıtına etki eden faktörleri saptamak için lojistik regresyon analizi uygulandı. Sonuçlar: Lojistik
regresyon analizi sonucunda yaş, cinsiyet, hastalığın başlama yaşı, hastalık süresi, eşlik eden tik bozukluğu, Y-BOCS
obsesyon, kompulsiyon, içgörü ve ciddiyet puanları değişkenleri arasında sadece içgörü puanının tedavi yanıtı üzerinde
etkili olduğu bulundu (p=0.038, %95 CI 0.225-0.957). Buna göre içgörü puanının 1 puan düşük olması tedaviye yanıt verme
olasılığını 2.5 kat artırıyordu. Tartışma: OKB’de tedaviye cevap ile ilişkili etkenler ile ilgili çalışmaların sonuçları yeterince
net değildir. Obsesif kompulsif bozuklukta tedavi başarısı ya da başarısızlığı ile ilişkili prognostik değişkenlerin doğru
şekilde tanımlanması tedavi programlarının gözden geçirilerek hastalara özgü farklı tedavi stratejilerinin uygulanmasına
yol açabilecektir.
Factors associated with treatment response in obsessive compulsive disorder
Objective: The objective of this study was to investigate potential factors that might influence response to
pharmacological treatment in obsessive compulsive disorder (OCD). Methods: A total of 60 patients with OCD diagnosed
according to DSM-IV criteria (39 women and 21 men) were included in the study. All patients were administered Yale-
Brown Obsessive Compulsive Scale (Y-BOCS). After clinical evaluation, the patients were started on pharmacotherapy
(28 patients on fluvoxamine 100-300 mg/day; 15 patients on fluoxetine 20-80 mg/day and 17 patients on sertraline 100-
200 mg/day) and dosages were adjusted according to clinical condition of the patients and adverse effects. Response to
treatment was evaluated with Clinical Global Impression Scale (CGI) at the end of 3rd [month] of treatment. Responders
were defined as those rated improved or very much improved on CGI. Results: A logistic regression analysis was done to
investigate factors that might influence treatment response. Age, sex, age at onset, duration of disorder, accompanying
tic disorder and obsession, compulsion, insight and severity scores on Y-BOCS were entered into logistic regression analysis, which revealed that among these variables, only insight score had influence on treatment response (p=0.038,
%95 CI 0.225-0.957). Every one-point decrease in insight score increased the likelihood of treatment response 2.5 fold.
Discussion: Results of studies about factors associated with treatment response are not conclusive. Identification of
prognostic factors associated with treatment success in OCD would lead to revision of treatment programs, resulting in
administration of patient-specific treatment strategies
