Majör depresyonda hassas C-reaktif protein düzeyleri ve tedavi ile ilişkisi

Amaç: Majör depresyonda C-reaktif protein (CRP)düzeyleri ile ilgili çalı şmaların sonuçları çelişkilidir. Hassas CRP yöntemi ile (high sensitive, hs-CRP) CRP ’deki daha küçük değişimler saptanabilir. Literatürde majör depresyonlu hastalarda hs-CRP yöntemi ile CRP düzeylerini araştıran her hangi bir çalışma yoktur. Bu çalışmada Majör depresyonlu hastaların hs-CRP düzeylerinin araştırılıp sağlğklı kontrollerle karşıyaştırılması amaçlanmıştır.
Yöntem: Klinik olarak majör depresyon tanısı konulan 23 kişi ve 26 sağlğklğ kontrol çalışmaya alındı. Depresyonun şiddeti Hamilton Depresyon Ölçeği’nin (HDÖ) 17 maddelik formu ile belirlendi. Hasta grubunda hs-CRP düzeyleri başlangıçta ve 8 haftalık antidepresan kullanımından sonra olmak üzere iki kez ölçüldü.
Bulgular: Majör depresyonlu hastaların tedavi öncesi hs-CRP düzeyleri 0.75 ± 0.34mg/dl, tedavi sonrası hs-CRP düzeyleri 0.76 ± 0.48 mg/dl olarak bulundu. Kontrol grubunun hs-CRP düzeyleri ise 0.71 ± 0.49 mg/dl olarak saptandı. Majör depresyonlu hastaların hs-CRP düzeylerinde kontrol grubuna göre istatistiksel olarak anlamlı fark yoktu. Ayrıca tedavi öncesi ve tedavi sonrası hs-CRP düzeylerinde de değişme saptanmadı (p>0.05).
Sonuçlar: Normal CRP ölçüm yöntemine göre serum CRP düzeyini 30 kat daha hassas düzeyde saptayabilen, hs-CRP yöntemi ile yapılan bu çalışmanın sonuçları majör depresyonlu hastalarda CRP düzeylerinin kontrol gruplarına göre farklı olmadığını ve antidepresan tedaviyle değişmediğini göstermiştir
The relationship between high sensitive C-Reactive protein levels and treatment response in patients with major depression
Objective: Many previous studies conducted about the levels ofC-reactive protein (CRP) in patients with ajor depression presentedcontradictory results which ight be a result of limitations inmeasurement of serum levels of CRP. However,even minoralterations of CRP levels can be determined with using recentlydeveloped high sensitive CRP (hs-CRP) measurement method. Tillnow, there have not been any studies investigating for CRP levels inmajor depressive patients using hs-CRP method. The aim of this studywas to determine serum hs-CRP levels in patients with majordepression who were treated with various antidepressants and tocompare with those of healthy controls.Methods: Twenty-three patients who are diagnosed clinically asmajor depression and 26 healthy controls were included in this study.The severity of the depression was quantified with 17-item HamiltonDepression Rating Scale (HAM-D). Serum hs-CRP levels weremeasured before and after antidepressant treatment of eight weeks.Results: Serum hs-CRP levels were found as 0.75 ± 0.34 mg/dl and0.76 ± 0.48 mg/dl before and after treatment respectively in majordepression group. The difference was not statistically significantaccording to paired samples T test (p>0.05). The level of healthygroup was 0.71 ± 0.49 mg/dl and the difference between controlgroup and major depression group was also insignificant (p>0.05).Discussion: The results of this study showed that serum CRP levels inmajor depressive patients as measured with hs-CRP measurementmethod – a 30 fold ore sensitive method for CRP serum levels than themethods used in previous studies-do not show any significantdifferences from healthy control group and do not change in quantitywith various antidepressant treatments
