Psikiyatri kliniğinde yatarak tedavi gören hastalarda alkol kullanımı

Amaç: “Psikiyatrik hasta” populasyonunda alkol kullanımının genel topluma göre yüksek olduğu bildirilmiştir. Bu çalışmanın
amacı “genel psikiyatri kliniğinde” yatarak tedavi gören hastalar arasında alkol kullanımı yaygınlığını araştırmaktır. Yöntem:
Çalışmaya psikiyatri kliniğinde yatarak tedavi gören 100 hasta alınmıştır. Hastaların psikiyatrik tanıları DSM-IV ölçütlerine göre
çalışmaya “kör” psikiyatrist tarafından konmuştur. Hastaların sosyodemografik özellikleri, alkol kullanım özellikleri belirlenmiş ve
Michigan Alkolizm Tarama Testi (MATT) ile değerlendirilmiştir. Sonuçlar: Çalışmaya 36 kadın (%36), 64 (%64) erkek hasta
alınmıştır. Yaş ortalaması 34.7±11.8, hastalık süresi 3.8±4.2 yıldır. DSM-IV ölçütlerine göre psikiyatrik tanılar depresif bozukluk
%36 (n=36), bipolar bozukluk-manik epizod %20 (n=20), şizofreni %20 (n=20), diğer (yeme bozukluğu, anksiyete bozukluğu
vs.) %29 (n=29) şeklindedir. Michigan Alkolizm Tarama Testi ortalaması 3.8±6.8’dir. Hastaların %35’i (n=35) hiç alkol
içmediklerini bildirmiş, MATT ile ise %47’sinin (n=47) sorunsuz alkol içtikleri (MATT puanı 0-4 arası), %18’inin (n=18) de
sorunlu içici oldukları (MATT 5’den yukarı) bulunmuştur. Tartışma: Psikiyatrik tanı ile yatan hasta populasyonunda “sorunlu”
alkol kullanımı oranı genel populasyondan yüksek görülmektedir. Tedavide böyle bir faktörün göz önünde bulundurulması faydalı
olabilir. Sonuçlar başka psikiyatrik tanılar ile tedavi gören hastalarda alkol kullanımının da “titizlikle” sorulması ve dikkate
alınması gerektiğini düşündürmektedir.
Alcohol use in psyhiatric inpatients
Objective: It is suggested that alcohol use is more prevalent among ‘psychiatric patients’ than it is in general population. The
purpose of this study was to find out the prevalence of alcohol use among the patients in inpatient service of a general
psychiatry clinic. Methods: To study, 100 patients who had been to inpatient our psychiatric clinics were included. The
patients were diagnosed according to DSM-IV by a psychiatrist who was ‘blind’ in the study. The sociodemographic
characteristics of the patients and the characteristics of their alcohol use were determined and evaluated by Michigan Alcholism
Screening Test (MAST). Results: The rate of female patients 36% (n=36) and male patients were 64% (n=64). The mean age
was 34.7±11.8 [year]s, the duration of their illness was 3.8±4.2 [year]s. The diagnoses based on DSM-IV were depressive disorder
36%(n=36), bipolar disorder I with manic episode 20% (n=20), schizophrenia 20% (n=20), and others (eating disorders,
anxiety disorders etc.) 29% (n=29). MAST score was 3.8±6.8, 35% of patients (n=35) indicated no alcohol use in their life,
47% of patients (n=47) with a MAST score of 0-4 point defined alcohol use without any problem and 18% of patients (n=18)
with a MAST score of over 5 point defined alcohol use with problems. Discussion: The rate of the alcohol use with problems
was among the psychiatric patients was found to be higher than it was in general population. This could be considered in and
could help in the treatment of the patients. Results suggest that alcohol use should be asked particularly and considered in
psychiatric patients who are treated for psychiatric diseases other than alcohol use.
