Uygunsuz antidiüretik hormon salıverilmesi sendromu (SIADH), pek çok nedenle birlikte karbamazepin, nöroleptikler, trisiklik antidepresanlar, metildopa ve son zamanlarda serotonin geri alınıp inhibe eden antidepresanlar gibi bazı psikotropik ilaçlara bağlı olarak da gelişebilen ve ciddi hiponatremiye neden olan bir sendromdur. Yetmiş yaşıda, bipolar bozukluk nedeniyle yatarak tedavi görmekte olan bir erkek hastaya manik direm tanısı ile günde 150-600 mg dozlarıda okskarbazepin (OKBZ) tedavisi uygulandı. OKBZ tedavisinin 6. haftasında hastada halsizlik, letarji, baş dönmesi, bulant ve hıçkırık gelişti. Hastada belirgin hiponatremi tespit edilirken (Na+ 106 mEq/l, K+ 4.4 mEq/l), renal fonksiyonlar normal bulundu. OKBZ sonlandırıldı ve hastaya günde iki kez furosemid 20 mg (iv) ve izotonik NaCl tedavisi başlandı. Na+ seviyesi bir hafta içinde normal değerlere ulaştı. Özellikle yaşlı hastalarda OKBZ ve diğer psikotropik ilaçlarla hiponatremi gelişme riski daha yüksektir. Yaşlı hastaların tedavisinde potansiyel olarak ciddi, ancak düzelebilen bu yan etki konusunda dikkatli olunmalıdır. Psikiyatrik hastalarda hiponatreminin belirtileri giden kalabilmektedir. Özellikle psikotropik ilaç kullanan yaşlı hastalarda serum elektrolitlerinin yakın takibi önleyici olabilir.

The syndrome of inappropriate antidiuretic hormone secretion (SIADH) may be associated with several situations and psychotropic drugs e.g., carbamazepine (CBZ), neuroleptics, tricyclic antidepressants, methyldopa and more recently selective serotonin reuptake inhibitors (SSRI) and can cause severe hypnonatremia. Here we report a case of SIADH with oxcarbazepine use in a 70- [year]-old male inpatient with bipolar disorder. He was given oxcarbazepine (OCBZ) 150-600 mg daily for manic state. At the sixth week of the OCBZ therapy, weakness, lethargy, dizziness, nausea and hiccups appeared in the patient. Significant hyponatremia was detected (Na+ 106 mEq?l, K+ 4.4 mEq?l), but renal functions were normal. OCBZ was stopped and he was given furosemide 20 mg t.i.d. intravenously and isotonic saline infusion. Na+ levels raised to normal values within a week. Elderly people are especially at increased risk for hyponatremia associated with OCBZ and other psychotropic drug use. Physicians treating elderly patients should be aware of this potentially serious, but reversible adverse effect. Vague signs of hyponatremia may not receive proper attentiom in psychiatric patients. Close scrutiny of serum electrolytes may be preventive especially for elderly patients having psychotropic medication.
