Fibromyaljili ve romatoid artritli hastalarda ağrı şiddeti ile anksiyete ve depresyon ilişkisi

Amaç: Fibromyalji (FM) ve romatoid artritin (RA) yüksek oranda çeşitli tıbbi ve psikiyatrik hastalıklarla birlikte olduğu
bilinmektedir. Bu amaçla psikiyatrik bozukluklardan depresyon ve anksiyetenin FM ve RA ile birlikteliğini ve bu hastalıkların RA
ve FM’deki ağrı şiddeti ile ilişkisini araştırmak istedik. Yöntem: 1990 American College of Rheumatology (ACR) kriterlerine göre
FM tanısı konan 37 kadın hasta, 1987 American Rheumatism Association (ARA) kriterlerine göre RA tanısı konan 30 kadın hasta
çalışmaya alındı. Her iki grubun depresyonları Beck Depresyon Ölçeği, anksiyeteleri Hamilton Anksiyete Ölçeği, ağrı şiddeti ise
Visual Analog Skala ile değerlendirildi. Bulgular: FM’li hastalarda ağrı şiddeti RA’li hastalara göre daha fazlaydı (p<0.001). FM’li
hastalarda anksiyete şiddeti, RA’li hastalara göre daha yüksekti (p<0.001). FM’li hastalarda depresyon RA’li hastalara göre daha
fazlaydı, FM’li 14 hastada (%37.8), RA’li 11 hastada (%36.6) depresyon mevcuttu, ancak istatistiksel anlamlılıkta değildi
(p>0.05). FM’li hastalarda depresyon şiddeti ile ağrı şiddeti arasında ise ilişki bulunamadı (p>0.05). RA’li hastalarda anksiyete
şiddeti ile ağrı şiddeti arasında ilişki bulunamazken (p>0.05), depresyon şiddeti ile ağrı şiddeti arasında istatistiksel olarak
anlamlı ilişki bulundu (p<0.001). Tartışma: Sonuç olarak FM’li hastalarda ağrı şiddeti RA’li hastalara göre daha şiddetliydi. FM’li
hastalarda anksiyete şiddeti RA’li hastalara göre daha fazlayken, depresyon oranlarında anlamlılık yoktu. FM’de anksiyete şiddeti
ile ağrı şiddeti ilişki gösterirken, RA’te depresyon şiddeti ile ağrı şiddeti arasında ilişki bulundu.
The relationship pain intensity, anxiety, and depression in patients with fibromyalgia and rheumatoid arthritis
Objective: Fibromyalgia (FM) and rheumatoid arthritis (RA) are frequently comorbid with various medical and psychiatric
disorders. The purpose of this study was to examine the relationship of comorbid among FM, anxiety, depression and was to
describe the relationship of RA with anxiety, depression. Method: Patients with FM were 37 women who each met the 1990
criteria of the ACR for FM. Patients with RA were 30 women who fulfilled the 1987 criteria of the ARA for RA. The patients with
FM and RA assessed their pain using a visual analog scale. The patients’ depressive symptoms were measured with the Beck
Self-Rating Depression Scale and anxiety symptoms were measured with the Hamilton Anxiety Scale. Results: Pain experience
was higher in the patients with FM than with RA (p<0.001). Anxiety scores were higher in the patients with FM than with RA
(p<0.001) and there was a positive correlation between the anxiety scores and the pain intensity (p<0.001). The patients (14
patients, 37.8%) with FM more depressive than the RA patients (11 patients, 36.6%), but a statistical difference between
depression score and pain intensity was not found (p>0.05). In the patients with RA, a positive correlation between anxiety
score and pain intensity was not found (p>0.05), but a positive correlation between depression score and pain intensity was
found (p<0.001). Discussion: The result show that FM patients have higher pain intensity than RA patients, and have higher
anxiety score than RA patients, but have not a difference depressive score. In the patients with FM a positive correlation was
found between anxiety scores and pain intensity, but in the patients with RA a positive correlation was found between
depression scores and pain intensity.
