Pozitif ve negatif belirtili şizofrenide hipotalamo-pituiter-tiroid eksen bulgularının sağlıklı bireylerle karşılaştırılması

Amaç: Pozitif (PB.) ve negatif belirtili şizofreni (NB.) arasında biyolojik ve nöroendokrin farklılıklar olduğu bilinmektedir. Bu çalışmada pozitif ve negatif belirtili şizofrenili hastaların hipotalamo- pitüiter tiroid (HPT) eksen bulgularının karşılaştırılması amaçlanmıştır.
Yöntem: Çalışmaya, 10 adet BP. ve 11 NB. hasta ile 21 adet sağlıklı birey (SB) alınmıştır. Grupların HPT eksen bulgularının karşılaştırılmasında, periferik tiroid hormon düzeyleri [serbest T3 (ST3), total T3 (TT3), serbest T4 (ST4) ve total T4 (TT4)] düzeyleri ile TRH'a TSH ve PRL yanıtları ve Dmax TSH düzeyleri kullanılmıştır.
Bulgular: HPT eksen bulguları açısından PB. ve NB. arasında anlamlı bir farklılık bulunamamıştır. PB. ve NB. gruplarının gerek periferik tiroid hormon düzeyleri, gerekse TRH'a TSH yanıtlarındaki körelme oranları (sırasıyla 40% ve 45%) kontrol grubuna göre (0%) anlamlı olarak yüksek bulunmuştur. Bazal PRL düzeylerinin ve TRH'a tüm PRL yanıtları açısından hasta grupları arasında anlamlı bir farklılık bulunamamış, fakat bu değerlerin kontrol grubunun değerlerinden anlamlı olarak yüksek olduğu saptanmıştır.
Tartışma: Bu bulgulara dayanarak, pozitif ve negatif belirtili şizofrenili hastaların HPT eksen bulguları açısından anlamlı bir farklılık olmadığı, dolayısıyla pozitif ve negatif belirtilerin fizyopatolojisinde, en azından, HPT eksen işlevleriyle ilişkili bazı nörotransmitter fonksiyonlarının benzer olduğu ileri sürülebilir.
Hypothalamo-pituiter-thyroid axis functions in positive and negative schizophrenia compared to healthy controls
Objective: It is known that there are some biological and neuroendocrine differences between positive and negative schizophrenia. This study was designed to compare with hypothalamopituitary- thyroid (HPT) axis findings in schizophrenic patients with positive and negative symptoms.
Method: 10 patients with positive symptoms and 11 with negative symptoms, and 21 healthy control (HC) subjects were included to the study. Thyroid hormones (free and total T3 and T4 levels), TSH responses to TRH and PRL, and Dmax TSH values were used to evaluate HPT axis functions.
Results: No significant differences were found between patients with positive and negative symptoms in terms of HPT axis functions. However, mean thyroid hormone levels, which remain in normal ranges, of the patient groups were significantly higher than those of the healthy controls. The rates of blunted TSH responses to TRH in patients with positive and negative symptoms are 40% and 45%, respectively, and these rates were significantly higher than those of the controls (0%). Basal PRL and PRL responses to TRH levels of the patient groups were similar, but higher than the those of healthy subjects.
Conclusions: These findings show that no significant differences in HPT axis functions between patients with positive and negative symptoms; and suggest that the mechanisms, which are related to pathophysiologia of the positive and negative symptoms, may be similar, at least, in terms of some neurotransmitters that related HPT axis functions.
