Akut manide lityum, karbamazepin ve sodyum valproatın klinik etkinliğinin karşılaştırılması: Bir ön çalışma

Amaç: Bu çalışmada, akut manik epizodda lityum, karbamazepin ve sodyum valproatın klinik etkinliğini incelemeyi amaçladık.
Yöntem: çalışmaya yatarak tedavi gören akut manili 15 hasta dahil edildi. Hastalar beşerli gruplara ayrılarak her bir gruba lityum, karbamazepin ve sodyum valproat tedavisi uygulandı. DSM-IV tan sistemi ile manik epizod tanı konulan hastalarda hastalığın ağırlık derecesi ve ilaçarı etkinliğini ölçmek için tedavinin başında ve 1. 6. haftalarıda Bech-Rafaelsen Mani Ölçegi (BRMAS), Kısa Psikiyatrik Derecelendirme ölçeği (BPRS) ve Klinik Global İzlenim Çizelgesi (CGI) uygulandı.
Bulgular: Tedavi öncesi ve birinci hafta sonucunda her üç ölçekte de gruplar arasında anlaml fark yoktu. Altıncı haftada sadece BPRS skoru lityum ve sodyum valproat verilen grupta anlamlı düzeyde düş- müştü (p<0.05).
Tartışma: Bu ön çalışmada, üç ilacın akut manik epizod tedavisinde ayn sürede aynı etkinliği gösterdiği bulunmuştur.
Comparison of the clinical efficacy of lithium, carbamazepine and sodium valproate in acute mania :Preliminary results
Objective: In this study, we aimed to investigate the clinical efficacy of lithium, carbamazepine and Na- valproate in acute mania episode cases.
Method: Fifteen patients admitted to the hospital with acute mania have been included in the study. Three groups (each consisting of five patients) were formed and treated with lithium, carbamazepine and Na-valproate as separate groups. The diagnosis of acute mania episode was made according to DSM-IV diagnosis criteria. The severity of the disease and the efficacy of the drugs measured by Bech-Rafaelsen Mania Scale (BRMAS), Brief Psychiatric Rating Scale (BPRS) and Clinical Global Impression Scale (CGI) before, at the first and the sixth week of the treatment.
Results: The scores before the treatment and in the first week were not significantly different between the three groups. In the sixth week, only BPRS scores were found to be significantly decreased in the lithium and Navalproate given groups (p<0.05).
Conclusions: In this pilot study, all the three drugs have been found to be almost equally effective in acute manic episodes with the same duration of treatment.
