Şizofreni ve nevrotik bozukluklarda testosteron düzeyleri ve testosteron düzeylerinin antipsikotiklere cevabı

Amaç: Bu çalışma şizofrenili hastaların serum testosteron düzeylerini nevrotik bozukluğu olan hastalar ile karşılaştırmak ve şizofrenili hastalarda serum testosteron düzeylerinin tedavi amacıyla kullanılmakta olan antipsikotik ilaçlara cevabını görmek üzere yapılmıştır.
Yöntem: Antipsikotiklerle tedavi edilen, DSM-IIIR'ye göre şizofreni tanısı konulan 30 erkek, yatan hastanın serum toplam testosteron, serbest testosteron, sex hormonu bağlayıcı globulin (SHBG) ve estradiol (E2) düzeyleri yatışlarının 1., 8., 15. ve 22. günlerinde ölçüldü. Aynı günlerde kısa psikiyatrik derecelendirme ölçeği (KPDÖ) ile hastalığın şiddeti de ölçüldü. DSMIII- R'ye göre nevrotik bozukluk tanısı konulan 30 erkek, ayaktan hastada ise aynı hormonların düzeyi bir kez ölçüldü.
Bulgular: şizofrenili ve nevrotik bozukluğu olan hastaların hormonal değerleri arasında istatistiksel olarak anlamlı farklılık bulunamadı. şizofrenili hastaların KPDÖ skorlarında tedavinin dördüncü haftasında anlamlı düşmeler saptandı. Hastalığın şiddetindeki bu düşmeye toplam testosteron düzeyi düşmesi ile verilen antipsikotiğin dozunda artış eşlik etmekteydi.
Sonuç: şizofrenili hastalarda antipsikotik ilaçlarla oluşturulan klinik iyileşmeye toplam testosteron düzeyinde düşme bulgusu eşlik etmektedir.
Serum testosterone levels in schizophrenia and neurotic disorders and testosterone responses to antipsychotics
Objective: The aims of this study have been to compare serum testosterone levels in patients with schizophrenia or neurotic disorders diagnosed according to DSM-III-R and to find the testosterone responses to antipsychotics in patients with schizophrenia.
Methods: Serum levels of total and free testosteron, sexhormone binding globuline and estradiol were measured once in 30 outpatients with neurotic disorders and were measured weekly for four weeks in 30 schizophrenic inpatients receiving antipsychotic treatment. BPRS scores were also obtained weekly in schizophrenia group.
Results: There was not any statistically significant difference in hormonal measures between two groups. Decrease in BPRS scores at fourth week was statistically significant. There was a positive correlation between the decrease in BPRS scores and total testosteron levels and a negative correlation between these two and mean daily antipsychotic dosage.
Conclusion: Decrease in serum total testosteron level is accompanied by treatment response to antipsychotics in patients with schizophrenia.
