Kronik şizofreni hastalarında klozapin ve sülpiridin etkinliğinin karşılaştırılması

Amaç: Klozapin ve sülpirid tedaviye dirençli şizofreni hastalarında etkili bulunmuştur. Bu çalışmada tedaviye dirençli şizofreni hastalarında klozapin ve sülpiridin etkinliğinin ve güvenirliliğinin karşılaştırılması amaçlandı.
Yöntem: DSM IV tanı ölçütlerine göre kronik şizofreni tanısı alan, klasik antipsikotik tedaviye dirençli, klinikte yatarak tedavi gören 36 hasta üzerinde yapıldı. Hastalara rasgele yöntemle 600-1600 mg/gün sulpirid ve 300-800 mg/gün arası klozapin verildi. Çalışmada Kısa Psikiyatrik Değerlendirme Ölçeği (KPDÖ) ve UKU yan etki ölçeği, kullanıldı. Tedavinin değerlendirmesi için tedaviye başlamadan KPDÖ ve tedavinin 15, 30 ve 60. günlerinde KPDÖ ve UKU yan etki ölçeği kullanıldı.
Bulgular: Tedavinin altmışıncı gününde klozapinle tedavi edilen hastaların KPDÖ puanları sülpirid grubuna kıyasla anlamlı derecede düşük bulundu. Klozapinle tedavi edilen grupta lökopeni ve agranülositoz görülmedi. Her iki grupta yan etkilerden dolayı tedaviyi bırakan olmadı.
Tartışma: Tedaviye dirençli şizofreni hastalarında klozapinin daha etkili bulundu.
A comparison of efficacy and safety of clozapine and sulpiride in treatment-resistant schizophrenia
Object: In this study, a comparison of efficacy and tolerability of clozapine and sulpiride on treatment-resistant-schizophrenia was aimed.
Method: Thirty-six in-patients with treatment-resistant schizophrenia, diagnosed according to DSM-IV diagnostic criteria, were given 600-1600 mg?day sulpiride and 300-800 mg?day clozapine, randomly. Patients were evaluated using Brief Psychiatric Rating Scale (BPRS) and UKU- Side Effects Scale on the 15th, 30th and 60th days of the study.
Results: The baseline and the end of the treatment period of Brief Psychiatric Rating Scale (BPRS) points of two groups were compared in the group that was treated with clozapine BPRS- points was significantly lower than the group that was treated with sulpiride. Leucopenia and agranulocytosis did not occur in the clozapine treated group. In the both groups there were no dropouts due to the side effects.
Conclusions: It is suggested that the clozapine is more effective than sulpiride on the treatment-resistantschizophrenia.
